About this file

This document is an appendix of required attachments to a proposal for the City of Ashland, Oregon's progressive design-build procurement for Project No. 2026-06: Water Treatment Plant PLC & HMI Upgrade. Proposers must submit responses to all evaluation criteria listed in Section 5 of the RFP, along with a completed proposal form, independent contractor certification, and acknowledgment of any addenda received. Key personnel information, including names, titles, and certification numbers, must be provided as required by Section 5 of the RFP. Proposers must be licensed by the State of Oregon Construction Contractors Board before signing the construction agreement, and any prime architect providing architecture or engineering services must be licensed as required under ORS 671.020, ORS 671.030, ORS 672.020, and ORS 672.060.

The proposal form requires proposers to provide company information including business name, Oregon CCB number, business address, telephone, email, and authorized signature. The City of Ashland encourages contracting with minority-owned, woman-owned, and emerging small business (MWESB) enterprises that are state certified, and proposers should indicate certification status in the applicable categories, including minority owned, woman owned, emerging small business, or veteran owned. Contractors must certify their independent contractor status under penalty of perjury, provide a W-9 form with correct taxpayer identification, and demonstrate they meet at least four criteria establishing legitimate independent contractor status, such as maintaining a separate business location, carrying commercial advertising, holding applicable insurance or performance bonds, and performing services for multiple clients.
