Appendix_1_-_EAP_RFP_GEO_Access_Census.xlsx

This is a geographic access census appendix document for the City of Phoenix's Employee Assistance Program Request for Proposal. The document serves as a supporting exhibit to the solicitation for comprehensive employee assistance services, including elder care, traumatic event counseling, and substance abuse professional services. The five-year contract is scheduled to commence on or about April 1, 2027, with the effective date established upon City Council award, signature, and recording by the City Clerk's department, whichever occurs later. The contract will terminate upon the earliest occurrence of reaching the end of the term including any extensions exercised or termination pursuant to the agreement's provisions.

The appendix document appears to contain geographic access and census data relevant to the service area and potential service population for the employee assistance program. This supporting information would inform offerors' understanding of the geographic scope, population distribution, and accessibility requirements necessary to develop competitive proposals. The census data serves to establish baseline information for service delivery planning, resource allocation, and geographic coverage expectations across the City of Phoenix's jurisdiction for the solicitation available through the City's Procurement Portal.
