About this file
This is an Annual Paperwork Packet for contractors bidding on 2026 contracts with the City of Toledo, Ohio. The packet contains four primary compliance certifications required for contract award: Equal Employment Opportunity (EEO) Certification, Living Wage Affidavit, Certification Regarding Debarment/Suspension, and Court-Ordered Child Support documentation. The EEO certification requires contractors to commit to non-discrimination in employment practices across hiring, compensation, training, and promotion, and mandates posting of equal opportunity notices. The Living Wage Affidavit requires contractors to pay employees a minimum of $17.45 per hour or 110% of the federal poverty level for a family of four (whichever is greater), or alternatively $20.63 per hour or 130% of federal poverty level if health benefits are not provided. Contractors must initial applicable paragraphs or claim exemptions for organizations with fewer than 25 employees, seasonal workers only, nonprofits, prevailing wage recipients, Community Development Block Grant recipients, or those designated for economic development purposes. The Debarment Certification requires prospective participants to certify they are not debarred, suspended, or ineligible from federal transactions and have not been convicted of fraud or criminal offenses related to public contracts within the preceding three years.
The Living Wage Affidavit requires contractors to provide twice-yearly notice to employees regarding living wage requirements and prohibits retaliation against employees reporting violations. All certifications must be signed and notarized where applicable by an authorized representative with contact information provided. The document notes that wage rates are subject to adjustment in January 2027 based on federal poverty line adjustments. Contractors are responsible for ensuring compliance extends to all subcontractors and vendors, and must report any changes in circumstances affecting the accuracy of submitted certifications to the City of Toledo immediately. These forms represent mandatory recurring submissions for all contractors on City of Toledo contracts advertised on or after March 13, 2026, unless information changes, requiring contractor-initiated updates to the City.
