About this file
This is a Required Affidavit for Bidders, Offerors and Contractors mandated by the Commonwealth of Kentucky under KRS 45A.110 and 45A.115. The affidavit must be submitted by any bidder, offeror, or contractor seeking to be awarded a contract or renew an existing contract with the state. An authorized representative of the contracting party is required to complete the attestation, obtain notarization, and submit the completed affidavit to the Commonwealth. The affidavit is effective for one year from the date of execution. No specific products, services, project timelines, response dates, or contract terms are detailed in this document, as it serves as a procedural requirement applicable across all state contracting opportunities.
The affidavit requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that contract award will not violate any provision of those laws. For purposes of the attestation, "knowingly" is defined as awareness or should-have-been awareness of a violation's existence. The Commonwealth retains the right to request an updated affidavit at any time. The form requires the contractor's name, address, and Commonwealth Vendor Code, along with the notary's certification and commission expiration date. No pricing, set-asides, or funding information is applicable to this procedural affidavit document.
