About this file
This is a Required Affidavit for Bidders, Offerors, and Contractors document issued by the Commonwealth of Kentucky in compliance with KRS 45A.110 and 45A.115. The affidavit is mandatory for any bidder, offeror, or contractor seeking to be awarded a state contract or for the renewal of an existing contract. An authorized representative of the contracting party must complete the attestation, obtain notarization, and submit the completed affidavit to the Commonwealth. The affidavit remains effective for one year from the date of execution. No specific products, services, project details, quantities, response dates, due dates, site visits, bidders' meetings, or award dates are contained within this document, as it serves as a general compliance form applicable across all state procurement activities.
The affidavit requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that awarding the contract will not violate any provision of such laws. "Knowingly" is defined as having actual awareness or should have been aware of the existence of a violation. The document includes fields for the bidder or offeror name, address, Commonwealth vendor code, signature line, notary information, and notary commission expiration date. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period. No pricing terms, set-asides, incumbents, funding sources, or other financial requirements are specified in this affidavit template.
