About this file
This is a Required Affidavit document issued by the Commonwealth of Kentucky for bidders, offerors, and contractors seeking state contracts. The affidavit, effective for one year from the date of execution, is mandated under Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. All contracting parties must submit a completed, notarized affidavit to be awarded a contract or to renew an existing contract. An authorized representative of the bidder or contractor must complete the attestation, obtain notarization, and return the document to the Commonwealth. The affidavit does not specify particular products, services, project timelines, quantities, response dates, or contract terms, as it serves as a blanket compliance requirement applicable to all state procurement activities.
The affidavit requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that contract award will not violate any campaign finance law provisions. For this attestation, "knowingly" is defined as awareness or constructive awareness of a violation. The document requires the contractor to provide identifying information including name, address, and Commonwealth vendor code. The affidavit must be notarized with the notary's signature, commission expiration date, and jurisdiction information. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period or renewal.
