About this file
This is a Required Affidavit for Bidders, Offerors and Contractors mandated by the Commonwealth of Kentucky under KRS 45A.110 and 45A.115. The affidavit applies to any bidder, offeror, or contractor seeking to be awarded a contract or contract renewal with Kentucky state agencies. An authorized representative of the contracting party must complete the attestation, obtain notarization, and submit the completed affidavit to the Commonwealth. The affidavit remains effective for one year from the date of execution. No specific project overview, quantities, response dates, or bidders' conferences are outlined in this document, as it serves as a general compliance requirement applicable across multiple contract opportunities rather than a solicitation for specific goods or services.
The attestation requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky campaign finance laws and that contract award will not violate any provision of such laws. "Knowingly" is defined as awareness or should-be awareness of a violation's existence. The Commonwealth reserves the right to request an updated affidavit at any time during the contract term. No pricing terms, set-asides, or funding information is contained within this document. This affidavit functions as a prerequisite compliance document rather than a pricing or funding instrument.

As a duly authorized representative for the Contractor, I swear and affirm under penalty of perjury, that that the Contractor has not knowingly violated campaign finance laws of the Commonwealth of Kentucky and that the award of a contract will not violate any provision of the campaign finance laws of the Commonwealth.
