About this file
This is an Annual Affidavit for Bidders, Offerors and Contractors required by the Commonwealth of Kentucky under KRS 45A.110 and 45A.115. The affidavit must be completed and submitted by any bidder, offeror, or contractor seeking to be awarded a contract or renew an existing contract with the state. An authorized representative of the contracting party is required to complete the attestation, obtain notarization, and return the executed affidavit to the Commonwealth. The affidavit is valid for one year from the date of execution. No specific products, services, quantities, response dates, or bidder conferences are addressed in this document, as it serves as a general compliance requirement applicable across all state contracting opportunities.
The attestation requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky campaign finance laws and that contract award will not violate any provision of those laws. "Knowingly" is defined as awareness or should-be awareness of a violation's existence. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period. The document requires completion of bidder or offeror name, address, Commonwealth vendor code if known, notarization by a qualified notary public, and signature with printed name, title, and date. No pricing terms, set-asides, funding sources, or other financial requirements are specified in this compliance document.
