About this file

This is a required affidavit form issued by the Commonwealth of Kentucky for bidders, offerors, and contractors seeking to be awarded state contracts or contract renewals. The affidavit, effective for one year from the date of execution, is mandated under Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. An authorized representative of the contracting party must complete the attestation, have it notarized, and submit it to the Commonwealth. The document serves as a compliance requirement prior to contract award or renewal.

The affidavit requires the contractor's authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky campaign finance laws and that the contract award will not violate any campaign finance law provisions. "Knowingly" is defined as awareness or should-be awareness of a violation's existence. The affidavit form includes fields for the bidder or offeror's name, address, Commonwealth of Kentucky vendor code, signature, printed name, title, and execution date. Notarization by a state notary is required, with the notary providing their signature, commission expiration date, and the state and county where the oath is administered. The Commonwealth reserves the right to request updated affidavits at any time during the contract period.
