About this file
This document is an Annual Affidavit for Bidders, Offerors, and Contractors required by the Commonwealth of Kentucky pursuant to KRS 45A.110 and 45A.115. The affidavit must be submitted by any bidder, offeror, or contractor seeking to be awarded a contract or renew an existing contract with the state. An authorized representative of the contracting party is required to complete the attestation, have it notarized, and submit it to the Commonwealth. The affidavit remains effective for one year from the date of execution, after which an updated affidavit may be required at the discretion of the Commonwealth.
The attestation requires the contractor's authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that awarding the contract will not violate any provision of those laws. "Knowingly" is defined as the bidder or offeror being aware or should have been aware of a violation's existence. The affidavit form requires the contractor to provide their name, address, Commonwealth of Kentucky vendor code if known, and the authorized representative's signature, printed name, title, and date. Notarization must be completed before a notary public, with the notary providing their signature, commission expiration date, and the state and county where the affidavit was sworn.
