About this file
This is a Required Affidavit for Bidders, Offerors, and Contractors issued by the Commonwealth of Kentucky pursuant to KRS 45A.110 and 45A.115. The affidavit is effective for one year from the date of execution and must be submitted by any bidder, offeror, or contractor seeking to be awarded a contract or to renew an existing contract with the Commonwealth. An authorized representative of the contracting party is required to complete the attestation, obtain notarization, and submit the completed affidavit to the Commonwealth prior to contract award. The form does not specify particular project timelines, response dates, or site visits, as it serves as a standard compliance document applicable across all state contracting opportunities.
The affidavit requires the authorized contractor representative to swear and affirm under penalty of perjury that the contractor has not knowingly violated campaign finance laws of the Commonwealth of Kentucky and that the contract award will not violate any provision of Kentucky's campaign finance laws. The term "knowingly" is defined as awareness or reasonable awareness of a violation's existence. The affidavit includes fields for the bidder or offeror name, address, Commonwealth vendor code, notary information, and commission expiration date. The Commonwealth retains the right to request an updated affidavit at any time during the contract term. This document represents a mandatory compliance requirement with no pricing, set-asides, or funding considerations, as it is solely a certification of campaign finance law compliance.
