About this file
This is an Annual Affidavit for Bidders, Offerors and Contractors required by the Commonwealth of Kentucky under KRS 45A.110 and 45A.115. The affidavit must be submitted by any bidder, offeror, or contractor seeking to be awarded a contract or to renew an existing contract with the state. An authorized representative of the contracting party is required to complete the attestation, obtain notarization, and submit the completed affidavit to the Commonwealth. The affidavit is effective for one year from the date of execution. There are no specific response dates, due dates, site visits, bidders' meetings, award dates, or contract term details provided in this document, as it serves as a general requirement applicable across all state contracts.
The affidavit requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky campaign finance laws and that the contract award will not violate any provision of those laws. "Knowingly" is defined as the bidder or offeror being aware or should have been aware of a violation. The Commonwealth retains the right to request an updated affidavit at any time. No pricing terms, set asides, incumbent information, or funding details are addressed in this document, as it functions solely as a compliance and attestation instrument for all contractors doing business with the state regardless of contract value or type.
