About this file
This is an Annual Affidavit for Bidders, Offerors, and Contractors required by the Commonwealth of Kentucky pursuant to KRS 45A.110 and 45A.115. The affidavit serves as a mandatory compliance document for any entity seeking to be awarded a state contract or contract renewal in Kentucky. The affidavit remains valid for one year from the date of execution and must be completed by an authorized representative of the contracting party, notarized, and submitted to the Commonwealth prior to contract award.
The affidavit requires the bidder or offeror to attest under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that awarding the contract will not violate any provision of those laws. The term "knowingly" is defined as awareness or should-be awareness of a violation's existence. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period. The document requires the bidder's signature, printed name, title, date of execution, bidder or offeror legal name, address, Kentucky vendor code if applicable, notary acknowledgment with commission expiration date, and specification of the state and county where notarization occurred.
