About this file
This is an Annual Affidavit for Bidders, Offerors and Contractors required by the Commonwealth of Kentucky pursuant to Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit must be completed and notarized by an authorized representative of any bidder, offeror, or contractor seeking to be awarded a contract or renew an existing contract with the Commonwealth. The affidavit is effective for one year from the date of execution. The form requires the contractor to attest under penalty of perjury that it has not knowingly violated Kentucky campaign finance laws and that contract award will not violate any provision of such laws. "Knowingly" is defined as awareness or should-have-been awareness of a violation's existence. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period.
The affidavit requires completion of standard contractor identification information including printed name, title, signature, date, bidder or offeror name, address, and Commonwealth of Kentucky Vendor Code if known. The document must be subscribed and sworn before a notary public, with the notary providing their signature, commission expiration date, state, and county of notarization. No pricing terms, funding amounts, or specific contract deliverables are addressed in this affidavit document, as it serves solely as a compliance certification requirement for participation in Commonwealth procurement processes.
