About this file

This Amendment #1 is issued by the Colorado Department of Natural Resources, Division of Reclamation, Mining and Safety for the Little Gypsum Mine Safety Closure Project (2017-0293). The project involves safety closure work at multiple mine features across San Miguel and Montrose counties on Bureau of Land Management property. The scope of work includes installation of mine safety closures using various methods such as backfilling, grated corrugated steel pipes (CSP), polyurethane foam, cable netting, and bulkheads across 44 distinct features. A pre-bid meeting was held prior to the bid deadline of Wednesday, July 12, 2017 at 4:00 p.m., with bid opening scheduled for Thursday, July 13, 2017 at 10:00 a.m. Sealed bids in single copy must be delivered by mail, hand delivery, or courier service to the Colorado Division of Reclamation, Mining and Safety at 1313 Sherman Street, Suite 215, Denver, Colorado 80203. Electronic and faxed submissions are not accepted. The bid schedule contains five line items: Mobilization/Demobilization, Project Safety, Health and Environmental Plan (SHEAP), Revegetation, Install Mine Safety Closures, and Additional Excavator Time (16 hours, not guaranteed).

The revised bid schedule requires contractors to price each mine closure feature individually, with total project funding sourced from OSM (Office of Surface Mining) and BLM (Bureau of Land Management) sources. Key amendments include mandatory radon monitoring using a SabreAlert Breathing Zone Monitor or equivalent, with daily measurements recorded and mitigation required for readings exceeding 0.1 Working Level; bat exclosure requirements for nine specified features with seven days and nights minimum placement prior to closure; priority completion by October 31, 2017 for four identified bat habitat features (416 A1, 450 A1, 455 A1, 462 A1); and revegetation of approximately 2.5 miles or 2.25 acres of equipment tracks. Additional specifications address props for supporting poor rock, scaling by MSHA-certified personnel, fall protection for ledges greater than 12 inches, and specific technical requirements for individual features including CSP substitution options and archaeological restrictions. Contractors must acknowledge receipt of the amendment and submit the revised bid schedule with all line items completed and all pages returned.

RE: Amendment No. 1 to 2017 - 0293

Thank you for your attendance at the Pre-Bid Meeting. The Bid Submittal will be due Wednesday July 12 at 4:00 p.m. The Bid Opening will be 10:00 am, Thursday, July 13. A REVISED Bid Schedule is attached to this amendment.

Please Note: Bids must be submitted and will only be accepted as sealed bids in single copy for the work described in the original bid document, and as detailed in this amendment and will be received until the date and time specified above at the Colorado Division of Reclamation, Mining and Safety, 1313 Sherman Street, Suite 215, Denver, Colorado 80203. Bids may ONLY be mailed, hand delivered, or sent via fed-ex, UPS, or other courier service. Faxed or emailed bids will NOT be accepted. Due to VSS Issues, the system is temporarily unable to accept bids electronically.

The following clarifications and amendments should be noted and incorporated into the bid documents:

1. Additional requirements to the Safety, Health and Environmental (SHEAP) Plan’s

Uranium Mine Site Work Safety Procedures are listed below.

The SabreAlert® Breathing Zone Monitor (BZM) radon detector or equivalent must be in operation and functioning during work being conducted at any mine entrance. If the BZM unit or equivalent is not in operation the CONTRACTOR must contact the PROJECT MANAGER. The highest radon daily measurement will be recorded on the Daily Work Site Logs in addition to the amount of exposure time each worker spends at the mine entrance.

The Radon and Gamma Log sheet must be completed and returned to the PROJECT MANAGER prior to final inspection of the Project.

Any radon reading greater than 0.1 Working Level must be mitigated using one of the following measures and method utilized recorded on the Daily Work Site Logs.

(1) substitution or elimination of the hazardous agent (e.g., via construction of ventilation barriers, installation of brattice cloth)

(2) engineering controls (e.g., local exhaust ventilation, process enclosure, dilution ventilation), http://mining.state.co.us/

(3) administrative controls (e.g., limiting time of exposure, employee training, work practices)

Please modify the reference in the SHEAP regarding the use of Self Contained Breathing Apparatus (SCBA) at sites with Radon monitoring exposure levels of greater than 5 Working Levels to address the above listed controls instead.

The CONTRACTOR will be responsible for the security and maintenance of the BZM or equivalent device while in their possession.

The radon and gamma initial log will be provided to the awarded CONTRACTOR to provide the radon and gamma data for the project. The radon and gamma log will include the highest radon readings for each feature and the final log must be submitted prior to approval of final invoice. NOTE: No initial radon measurements were collected for this project. For all features which will be mechanically backfilled using a piece of equipment fitted with an enclosed cab and functional air conditioner, radon progeny measurements are not required.

2. Note that Props may be necessary and required to support incompetent or poor back at many locations.

3. Scaling of loose, fractured or broken rock must be accomplished where necessary by a competent person, preferably someone trained to do so and certified by MSHA.

4. Anchoring and fall protection is required for all activities taking place at or adjacent to any ledge greater than 12 inches in height. The use of anchoring and PPE, including ropes and harnesses, must be addressed in the required SHEAP submittal.

5. In the event fire restrictions are in place on U.S. Bureau of Land Management properties at the time of construction – All applicable restrictions and practices prohibited in the field will apply unless fire conditions change. PROJECT MANAGER will notify CONTRACTOR if conditions warrant any restrictions.

6. The following clarifications are made to the following features:

All features with bat grates will include an access door per Standard Drawings 5 and 6 sheet 2, with the exception of features 458 S1 and 956 A2. All 36” X 20’ CSP’s can be substituted with 48” X 20’ – contractor’s option.

#3040 S1 – Grate only, not enough backfill material for backfill. Excavate timber frame and pin grate to bedrock. Wood from timber frame can be stacked on-site.

Features: 416 A2, 416 A3, 451 A1, 454 A1, 464 A1, - Backfill trench or pit areas with on-site waste rock to ground surface to mitigate high wall danger. Feature 416 A2 use topsoil stockpile for final surface seedbed and also include excavating a 50 foot long, 3 foot wide by 2 feet deep trench to intercept surface runoff.

#452 A1 – Archaeological restrictions include both conical waste rock piles at the site.

Additional backfill must be trammed from waste rock pile 300’ to the northwest.

#455 A1/A2 – Install CSP’s per specifications, band the two CSP’s for 455 A1 and A2 with Y pipe and band to an additional 10’ CSP with bat grate and access door. Backfill CSP’s in trench area with waste rock material. Include additional costs on Bid Schedule on line for feature #455 A1.

#461 A1 and A2 – Contractors option – CSP as specified in original bid document or Bulkhead with CSP per SWS 14 with bat grate and access door, Drawing 6 sheet 2.

Circle option preferred on REVISED Bid Schedule.

#Features 464 A1, 465 A1, 465 A2 - remove and properly dispose of T-posts and barbed wire.

7. The Mine Site Field Forms of all the mine sites will be emailed by the PROJECT

MANAGER and mailed with amendment. (NOTE: do not refer to closure method on MSFF, use the closure method on the revised bid schedule.) The Draft Bid Specification and pictures of the strapping and wire rope netting required are attached. Please note, alternative methods of wire rope netting must be approved by Project Manager prior to installation.

8. For all features where a Bat Exclosure will be constructed prior to closing the feature

(3040 A1, 3040 S1, 3041 S1, 3045 A1, 3048 P1, 451 A1, 454 A1, 458 P1, 463 A2) change the length of time for the bat exclosure to be left in place from a minimum of three days and nights prior to the start of mine safeguarding construction to seven days and nights.

9. Features 416 A1, 450 A1, 455 A1, 462 A1 were identified as priority bat habitat and closures must be completed by October 31, 2017.

10. Revegetation costs will include revegetation of equipment tracks on non-established two-tracks. Estimated length of 2.5 miles or 2.25 acres.

All terms and conditions of the Invitation for Bid other than as changed by this amendment remain the same.

Receipt and acceptance of this amendment shall be acknowledged on the bid form and by submitting the Revised Bid Schedule.
