AMENDMENT OF SOLICITATION/MODIFICATION OF CONTRACT - Attachment to SF 30 2 3

ISSUANCE DATE: 05/27/2010 PROJECT NAME: McCloud Office Building 2

AMENDMENT NUMBER: 0002

List below (or on attached sheet, if necessary) each proposed modification identified by Item Number, location of station, etc., giving the reasons for the modifications.

SUMMARY OF CHANGES:

A. Reference SF 1442, page 1 of 43, block 13 a: Offers due date is hereby changed from 06/01/2010 at 2:00 pm to 06/07/2010 at 2:00 pm.

B. The purpose of this amendment is to provide the below Questions and Answers (Q&A).

Q1. Reference Spec section 2411, 3.02 HAZARDOUS MATERIALS: Does the USFS intend to abate the ACBMs itself (per the conversation at the walk through) and, if it does not, will the Contractor be given a change order to abate them as part of the project?

A1. All known asbestos will be removed from the Fuels Office, Garage and Conference Room prior to the start of the contract by Forest Service approved personnel. Any other hazardous materials that need abatement including lead paint is part of the overall contract and is not intended to be under a separate contract. The list of prospective bidders and those that signed up at the pre-bid meeting are available at FedBizOpps if you would like to offer them your services if they are the winning bidder. It is not for us to suggest or say who they need to subcontract with.

Q2. Are the light poles shown on E5.0 part of Option A (this question from a sub)? Unless I hear differently from you, I will assume they are part of the base bid?

A2. The light poles near the parking lots are part of the base bid.

Q3. What work is intended in the areas of stamped/colored concrete if Option C is not awarded (plansheet C2.0)? Unless I hear differently from you, I will assume that no work is required there?

A3. If the stamped/colored concrete is not awarded, only rough grading would be required. This would be necessary to ensure the surrounding benches and sidewalks would be at the correct elevation. No finish grading or concrete would be placed in this area.

Q4. Is the bench wall on C2.0 part of Option C or the base bid? Unless I hear differently from you, I will assume it is part of the base bid?

A4. The bench wall is part of the base bid.

Q5. Where are the 2 drainpipes that lead to the bioswale to be daylighted if Option B is not awarded (sht C2.0, bottom left)? Unless I hear differently from you, I will choose to daylight them south of the parking lot at a spot of our choosing.

A5. The storm drains are to be at the design elevations, grade and alignment, as shown on the plans. If the bioswale is not awarded, then the lines will terminate a minimum of 5' past the edge of the parking lot. The ends will be capped with temporary caps and their locations marked.

Q6. Please confirm that all capping above the lava stone is poured-in-place concrete, not lava stone?

A6. The elevations call out note #4 which states stone veneer with C.I.P. concrete cap. C.I.P. stands for Cast In place, so yes it is poured in place and not lava stone.

Q7. I am looking at the third page of the SF-1442 form and am unsure what to fill in on this sheet. It seems to be a re-hash of Section B. Does this form need to be filled out by the GC? If so, are there instructions available somewhere for this form?

A7. SF 1442, page 3 of 43 requires no action--Offerors are required to complete Section B.

Q8.

A8. One four foot length of sidewalk to be stamped should be sufficient and if approved can be left in place.

Q9. There is a discrepancy on sheet e-5 and sheet c-2, 4 light poles or 3 light poles?

A9. Lights: Both sheet C2 and E5 show 4 lights, however the note for the call out on sheet E5 states "typ of 3". This note is incorrect. There are 4 lights required. Three are located at the parking lot south of the new building and one light is located near where the sidewalk from the existing trailers meet the parking lot to the north. All four locations are shown graphically on the plan sheets.

Q10. Please provide confirmation as to whether the wiring in the new office will be in either romex or mc cable?

A10. Electrical wire: Specification 16050, section 2.05(G) states that NM will only be allowed when approved by the Contracting Officer. Romex is allowed for concealed spaces only.

C. Submit all questions in writing via e-mail to the following technical POC: jstreuber@fs.fed.us

D. All other terms and conditions remain unchanged.
