The Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects (FP-03 U.S. Customary Units), 2003 are included by reference. The requirements contained in these specifications are hereby made a part of this solicitation and any resultant contract.

101.01 Meaning of Terms.

101.04 Definitions.
Delete the following definitions and substitute the following:
Bid Schedule--The Schedule of Items.
Bridge--No definition.
Contractor--The individual or legal entity contracting with the Government for performance of prescribed work. In a timber sale contract, the contractor is the “purchaser”.
Culvert--No definition.
Right-of-Way--A general term denoting (1) the privilege to pass over land in some particular line (including easement, lease, permit, or license to occupy, use, or traverse public or private lands), or (2) Real property necessary for the project, including roadway, buffer areas, access, and drainage areas.
Add the following:
Adjustment in Contract Price--“Equitable adjustment,” as used in the Federal Acquisition Regulations, or “construction cost adjustment,” as used in the Timber Sale Contract, as applicable.
Change--“Change” means “change order” as used in the Federal Acquisition Regulations, or “design change” as used in the Timber Sale Contract.
Design Quantity--“Design quantity” is a Forest Service method of measurement from the FS-96 Forest Service Specifications for the Construction of Roads and Bridges. Under these FP specifications this term is replaced by the term “Contract Quantities”.
Forest Service--The United States of America, acting through the Forest Service, U.S. Department of Agriculture.
Neat Line--A line defining the proposed or specified limits of an excavation or structure.
Pioneer Road--Temporary construction access built along the route of the project.
Purchaser--The individual, partnership, joint venture, or corporation contracting with the Government under the terms of a Timber Sale Contract and acting independently or through agents, employees, or subcontractors.
Protected Streamcourse--A drainage shown on the plans or timber sale area map that requires designated mitigation measures.
Road Order--An order affecting and controlling traffic on roads under Forest Service jurisdiction. Road Orders are issued by a designated Forest Officer under the authorities of 36 CFR, part 260.
Schedule of Items--A schedule in the contract that contains a listing and description of construction items, quantities, units of measure, methods of measurement, unit price, and amount.
Utilization Standards--The minimum size and percent soundness of trees described in the specifications to determine merchantable timber.

102 - Bid, Award, and Execution of Contract 102 Bid, Award, and Execution of Contract Delete Section 102 in its entirety.
103 - Scope of Work Deletions Delete all but subsection 103.01 Intent of Contract.
The project consists of the removal of an existing concrete slab bridge and the construction of a new 48’ span by 35’ wide concrete double tee bridge with a driven pile foundation. The bridge is located on Forest Service Road No. 618, milepost 0.10, at the Junction Wash crossing. Improvements consist of, but are not limited to: structure excavation, placement of riprap, installation of steel H piling, cast-in-place concrete pile cap, cast-in-place concrete end diaphragms and wingwalls, installation of prestressed concrete double tee beams, concrete bridge barrier and approach guardrail, embankment construction of the roadway approaches, and supplying and placing new asphaltic concrete surfacing.
The project is located in the Red Rock Ranger District of the Coconino National Forest, approximately 0.1 miles southeast of the Sedona Interchange on Interstate 17 in Yavapai County, Arizona (Section 17, Township 15 North, Range 6 East). Refer to the location map on the title sheet of the plans.
104 - Control of Work 104 Control of Work.
Delete sections 104.01, 104.02, and 104.04.
Add the following subsection:
104.06 Use of Roads by Contractor
The Contractor is authorized to use roads under the jurisdiction of the Forest Service for all activities necessary to complete this contract, subject to the limitations and authorizations designated in the Road Order(s) or described in the contract, when such use will not damage the roads or national forest resources, and when traffic can be accommodated safely.
105 - Control of Material
105.02 Material Sources.
Add the following before the first paragraph.
Sources have been designated for this project as follows:
Crushed Aggregate Source – Contractor Furnished Riprap Source – Government Furnished - A material source for riprap is available approximately one mile from the project site at the Red Tank Pit. The source is located in Section 16, Township 15 N, Range 6 E and is shown on the cover sheet of the Plans.
Roadway Embankment Source – Government Furnished - A material source for roadway embankment is available approximately one mile from the project site at the Red Tank Pit. The source is located in Section 16, Township 15 N, Range 6 E and is shown on the cover sheet of the Plans.
Waste Site – Government Furnished - A waste site for unsuitable material is available approximately one mile from the project site at the Red Tank Pit. The site is located in Section 16, Township 15 N, Range 6 E and is shown on the cover sheet of the Plans.
Materials Stockpile Site – Government Furnished – The Contractor will not be allowed to stockpile material, including riprap, crushed aggregate, and bridge components at the project site, adjacent to the existing bridge. Materials must be stockpiled at the site located by the CO. The site is located in Section 16, Township 15 N, Range 6 E and is shown on the cover sheet of the Plans.
Add the following to section (b) Contractor-located sources:
Prior to any pit development or use of materials, the Contractor shall provide to the CO a State certification that the pit site is free of noxious weed as listed on the "All States Noxious Weeds List”.
106 - Acceptance of Work
106.07 Delete
Delete subsection 106.07.
107 - Legal Relations and Responsibility To the Public

107.10 Environmental Protection.
Add the following:
Design and locate equipment repair shops, stationary refueling sites, or other facilities to minimize the potential and impacts of hazardous material spills on Government land.
Before beginning any work, submit a Hazardous Spill Plan. List actions to be taken in the event of a spill. Incorporate preventive measures to be taken, such as the location of mobile refueling facilities, storage and handling of hazardous materials, and similar information. Immediately notify the CO of all hazardous material spills. Provide a written narrative report form no later than 24 hours after the initial report and include the following:
· Description of the item spilled (including identity, quantity, manifest number, and other identifying information).
· Whether amount spilled is EPA or state reportable, and if so whether it was reported, and to whom.
· Exact time and location of spill including a description of the area involved.
· Containment procedures.
· Summary of any communications the Contractor had with news media, Federal, state and local regulatory agencies and officials, or Forest Service officials.
· Description of clean-up procedures employed or to be employed at the site including final disposition and disposal location of spill residue.
When available provide copies of all spill related clean up and closure documentation and correspondence from regulatory agencies.
The Contractor is solely responsible for all spills or leaks that occur during the performance of this contract. Clean up spills or leaks to the satisfaction of the CO and in a manner that complies with Federal, state, and local laws and regulations.
108 - Prosecution and Progress 108 Delete
Delete Section 108 in its entirety.
109 - Measurement and Payment 109 Deletions
Delete the following entire subsections:
109.06 Pricing of Adjustments.

(b) Contract quantity.
Add the following:
Contract quantities will be adjusted only when there are errors in the original design of 15% or more.
Change the following:
“(b) Cubic yard” to “(c) Cubic yard”.
Add the following definitions:
(p) Thousand Board Feet (Mbf). 1,000 board feet based on nominal widths, thickness, and extreme usable length of each piece of lumber or timber actually incorporated in the job. For glued laminated timber, 1,000 board feet based on actual width, thickness, and length of each piece actually incorporated in the job.
(q) Actual Quantities (AQ). These quantities are determined from measurements of completed work.
151 - Mobilization
151.01 Description
Add the following requirements:
Noxious Weed Control Requirements:
All construction equipment to be used at the job site shall be cleaned and certified free of noxious weeds and their seeds prior to entrance on to forest lands, whether they are owned, leased or borrowed by the Contractor or any subcontractors. “Construction equipment” does not include personal vehicles or support equipment that remains on the main travel route.
Cleaning shall occur off forest land.
Construction equipment shall be visually inspected by the COR and each one certified in writing to be reasonably clean and weed free. Inspections will take place at a forest administrative site with the Contractor providing at least 24 hours notice. The location of the administrative site is to be agreed upon in advance between the COR and the Contractor or Subcontractor. Equipment is expected to proceed directly to the job site following the inspection and to not leave the designated weed management district wherein the job site exists.
Certification shall remain valid for each identified piece of equipment for the duration of the specified project and only as long as the equipment remains at the job site or within the designated weed management district. Equipment leaving the designated weed management district will need to be recertified as weed free before being allowed to return to the job site.
151.03 Payment
Add the following:
Include all costs associated with the cleaning of equipment in the unit price for Mobilization.
152 – Construction Survey And Staking
152.03 Survey and Staking Requirements.
Add the following:
Conduct all construction staking under the direction of a licensed professional engineer or land surveyor who is closely associated and familiar with construction staking.
153 – CONTRACTOR QUALITY CONTROL

155 – Schedules For Construction Contracts 155 Delete
Delete Section 155 in its entirety.
156 - Public Traffic
156.03 Accommodating Traffic During Work.
Delete the first paragraph and replace with the following:
Accommodate traffic according to the requirements set forth below, MUTCD, and Section 635. Submit a traffic control plan for acceptance at least 30 days before intended use.
Add the following project specific requirements:
The Contractor will not be allowed to close Road No. 618 at Junction Wash to through traffic. The Contractor shall provide a bypass road and culvert or bridge, as specified below, to accommodate through traffic.
It is the responsibility of the Contractor to schedule delivery of materials, equipment and all necessary supplies to the sites, to ensure that the bridge can be substantially completed within the specified contract time. The Contractor shall give the CO at least two weeks advanced written notice prior to any road closure. The Contractor shall be responsible for providing, installing, maintaining, and removal of warning signs and type 3 barricades required for the road closure period in accordance with MUTCD as submitted and approved in the traffic control plan.
Temporary bypass culvert or bridge shall meet AASHTO LRFD Bridge Design Specifications, 4th Edition, 2007, with all current interim specifications and the following criteria:
1. Hydrologic Flows

2. Culvert:
The culvert supplied shall be designed to carry legal loads and shall be placed with adequate cover.
3. Bridge:
a. Load Rating:
AASHTO HL-93 with Load Rating completed by a
Professional Engineer registered in the State of
Arizona.
b. Traffic:
Single Lane; 14-foot curb-to-curb width.
c. Railing:
15-inch-high curbs (no approach rail).
4. Location:
The location shall be at the approximate location as shown on the plans and as approved by the C.O. Land disturbance shall be kept to a minimum.
5. Contractor shall be responsible for construction and maintenance of temporary bypass culvert/bridge and approach roads to safely pass public traffic through the road. Contractor shall prevent sediment from being introduced into the channel during construction and maintenance of approach roads for the temporary bypass.
6. Approach road shall be constructed with a minimum width of 12 feet and shall be surfaced with a minimum of 2 inches of road surfacing.
7. Contractor shall be responsible for removal of temporary bypass culvert/bridge, approach roads, and restoration of the site.
8. No railroad flat cars will be permitted for use as the temporary bypass bridge.
9. Submittals:
Load rating, design calculations, and working drawings showing the bridge length and width, location of bents, and details of the bridge curb/rail. Submit for acceptance at least 30 days before intended use.
Delete the following from the last sentence of the last paragraph:
according to Subsection 106.07(b)
156.08 Traffic and Safety Supervisor.
Delete this subsection in its entirety.
157 - Soil Erosion Control
157.02 Materials.
Add the following:
Provide bales, wattles, logs and rolls from a certified noxious weed free source.
157.03 General.
Delete the first two paragraphs and replace with the following:
Submit an Erosion Control Plan detailing permanent and temporary control measures to minimize erosion and sedimentation during and after construction in accordance with the plans and storm water permits. Do not modify the type, size, or location of any control or practice without approval. Submit the erosion control plan proposal at least 7 days before operations begin to the CO for approval.
Reflect in the Erosion Control Plan special concerns and measures necessary to protect resources and government improvements. Include:
(a) The construction activities and sequence of implementation relating to specific erosion control measures.
(b) The location and type of permanent controls to be implemented during construction.
(c) The location and type of temporary controls to be implemented during construction.
(d) For work in stream channels with running water a detailed dewatering plan.
(e) For work in stream channels without flowing water describe level of ground and vegetative disturbance and measures to reduce potential sediment delivery.
(f) Description of the monitoring plan.
Arizona Department of Environmental Quality (ADEQ) Requirements
This project will disturb less than one acre; however, the Contractor shall submit a notice of intent (NOI) and shall prepare a Storm Water Pollution Prevention Plan (SWPPP) in accordance with the requirements of the ADEQ. The SWPPP shall be submitted to the CO prior to starting construction. It may be submitted in conjunction with or as a supplement to the Erosion Control Plan described above.
Add the following to the third paragraph:
Upon completion of construction at the site, remove all temporary erosion control devices, dewatering materials and equipment from Government property.
203 – Removal of Structures and Obstructions
203.05 Disposing of Material.
Modify this subsection as follows:
(a) Remove from project. Add the following:
All materials designated for removal become property of the Contractor and are to be disposed of by removing off of Forest Service Lands in accordance with all local, state, and federal requirements.
(b) Burn. Delete this section.
(c) Bury. Delete this section.
Add the following:
(e) Construction slash may be chipped and used onsite or it may be disposed of at the waste site described in FSSS 105.
Construction slash shall not be placed in the roadway borrow ditches.
204 - Excavation and Embankment
204.14 Disposal of Unsuitable or Excess Material.
Delete the text of the first paragraph and substitute the following:
Dispose of unsuitable or excess material at designated sites or as directed by the CO.
208 - Structure Excavation And Backfill For Selected Major Structures
208.04 General.
Add the following:
Use all suitable material in the construction of the roadway embankment and for structural backfill. Field drain and dry excessively wet material that is otherwise suitable for embankment before placement. Place and shape unsuitable or excess material in the designated sites or as directed by the CO.
251 - Riprap
251.01 Description.
Delete the first sentence and add the following:
This work consists of furnishing, hauling, and placing riprap for bank protection, slope protection, drainage structures, erosion control, and other features shown on the plans.
251.07 Acceptance.
Delete sampling and testing requirements of Table 251-1 Delete the fourth sentence and add the following:
Structure excavation and backfill will be evaluated under section 208.
Delete Table 251-1.
251.09 Payment.
Add the following:
The cost of excavation, embankment, and haul required for placement of riprap is incidental to the riprap pay item.
551 – DRIVEN PILES
551.01 Add the following:
The geotechnical report is included in Appendix A. The geotechnical report is for the Contractor’s information only. There is no expressed or implied guarantee that the data included, or the interpretations thereof, represent either extreme, average, or the only soil and groundwater conditions to be found at the project site.
553 – PRESTRESSED CONCRETE
553.05 Method Approval.
Add the following:
Precast/Prestressed concrete members must be manufactured by a plant with the following certifications:
· Precast / Prestressed Concrete Institute (PCI)
· Bridge 4, Prestressed Deflected-Strand Bridge Beams (superstructure).
· National Precast Concrete Association (NPCA)
· Precast and Prestressed
Submit a copy of the transmittal letter of the latest PCI or NPCA inspection/certification with the shop drawings.
618 – CONCRETE BARRIERS AND PRECAST GUARDWALLS
618.02 Material
Change the concrete class reference to “Concrete class C(AE)”.
622 - Rental Equipment
622.02 Rental Equipment.
Delete the first two sentences of the first paragraph and add the following:
Hydraulic Excavator. Provide a track mounted hydraulic excavator. Excavator shall be no more than 15 years old and shall be reasonably free of leaks and other mechanical deficiencies. Bucket must be equipped with a hydraulic "thumb". Contractor shall supply an excavator of a size capable of performing the excavation needs for the project, i.e. structure excavation, riprap construction, etc., or other work that may be directed under this section. Submit the model number of the equipment to the CO for approval before delivery to the project site. Contractor is required to get approval of the size of the Hydraulic Excavator from the Contracting Officer prior to any work taking place.
625 – TURF ESTABLISHMENT
625.04 Preparing Seedbed.
Delete entire subsection.
625.05 Watering
Delete entire subsection and replace with the following:
Moisten seeding areas before seeding.
625.06 Fertilizing.
Delete entire subsection – There will be no application of fertilizer.
625.07 Seeding.
(a) Dry method. Delete the third sentence.
Add the following after subsection (b).
Seed Mix. Seed mix shall be pre-mixed. Furnish and apply the following kinds and amounts pure live seed:

Seed Certification
Certified, blue-tagged seed shall be used where a name variety or cultivar is specified. Blue tags that are removed to mix or spread the seed will be saved and provided to the Forest.
Submit lab tests for each species.
All seed purchased will be certified free of seeds from weeds listed on the current State of Arizona list.
Pounds of seed to be furnished per acre shall be obtained by dividing the pounds of pure live seed required per acre by the product of the percent purity and percent germination.
Example: (5 lbs. pure live seed/acre) = 6.55 lbs.
(0.90 x 0.85) commercial seed/acre where: purity = 90%; germination = 85%.
625.08 Mulching.
Delete the entire subsection and replace with the following:
Mulching:
Mulch material shall be weed free straw and shall be applied at a uniform rate to adequately cover the seed. The weed free straw shall be from a grain crop such as barley straw, wheat straw or oat straw.
Mulch will be applied in a one-step operation with the seed at the rate of 1 Ton per Acre.
725 - Miscellaneous Material Add the following section:
725.40 High-Strength Nonshrink Grout.
Furnish grout that is packaged and ready for use with the addition of water at the construction site. Ensure that each bag is stamped to show the last date on which it may be used. Use grout that consists of a hydraulic cementitious system, graded and processed natural fine aggregate, and additional technical components such that the product meets the following conditions:
(a) Free of inorganic accelerators, including chlorides.
(b) Free of oxidizing catalysts.
(c) Free of gas-producing agents.
(d) When mixed to 130 percent flow on flow table (ASTM C 230 at 10 drops), it does not reduce in linear dimension when tested in accordance with ASTM C157. Take measurements at 72 hours and 7 days.
(e) Produces no bleeding for the first 2 hours after mixing when mixed to 130 percent flow on flow table (ASTM C 230 at 10 drops), as tested in accordance with ASTM C232.
(f) Has a minimum strength as follows when tested in accordance with ASTM C 109:
(1) After 72 hours, 25 MPa (3500 psi).
(2) At 7 days, 40 MPa (6000 psi).
(3) After 28 days, 50 MPa (7200 psi).
Provide performance characteristics at 115 to 120 percent flow on flow table (ASTM C 230 at 10 drops).
(g) It must be designed, as stated by the manufacturer, to be mixed, placed, and cured at atmospheric temperatures of 5O C to 30O C (40O F to 85O F). Submit products proposed for use for approval by the CO and accompany them with manufacturer’s submittals substantiating all requirements in this subsection, including graphs or charts showing the time, temperature, and humidity requirements for curing to achieve the specified grout strengths; and recommendations for storage, mixing, application, and curing procedures.
As specified as grout on the contract plans, the following products meet the above requirements.
1. MASTERFLOW 928 GROUT, manufactured by Chemrex.
2. CRYSTEX GROUT, manufactured by L&M Construction Chemicals, Inc.
3. CEMENT ALL, manufactured by CTS Cement Manufacturing Co.
4. SURE-GRIP HIGH PERFORMANCE GROUT manufactured by Dayton Superior.
725.41 Mortar.
Furnish mortar that is packaged and ready for use with the addition of water at the construction site. Ensure that each bag is stamped to show the latest date on which it may be used. Use mortar that consists of a cementitious system made up of:
(a) Natural aggregate, 3/8” maximum size, that meets the requirements specified in ASTM C 33 except for grading. Accomplish grading by blending sieve sizes to obtain the optimum density.
(b) Metallic aggregate free from nonferrous material, soluble alkaline compounds, and visible rust.
(c) Water reducers, workability agents, air-entraining agents, and catalysts.
Blend the materials to minimize bleeding, increase workability, resist exposure to freeze-thaw cycles and deicing salts, and prevent shrinkage within and at the perimeter of the patch, keyway, or other area to be filled.
Ensure that the minimum compressive strength of the mortar, as tested by ASTM C 109 for a 3” slump, is:

· 7-day.............................................................................................60 MPa (8500 psi)
· 28-day........................................................................................70 MPa (10,000 psi)
Ensure that the durability of the products when tested at 300 cycles, ASTM C 666, procedure A is:

Ensure that the scaling resistance has a rating of 3, Moderate Scaling, after 50 cycles when tested in accordance with ASTM C 672.
Provide certification from the manufacturer that the product is compatible for work that is 1” or more in depth and more than 1” in width; and where the mixing, placing, and curing temperatures may range from 5O C to 30O C (40O F to 85O F).
Submit products proposed for use to the CO for approval, and accompany them with the manufacturer’s submittals substantiating all requirements in this section, including (1) graphs or charts showing the time, temperature, humidity, and curing requirements to achieve mortar strengths equal to the adjacent concrete; and (2) complete recommendations for storage, mixing, application, and curing procedures.
As specified as mortar on the contract plans, the following products meet the above requirements.
1. EMACO T425 or T430, manufactured by Chemrex.
2. RAPID SET DOT REPAIR MIX manufactured by CTS Cement Manufacturing Co.
3. SURE-GRIP HIGH PERFORMANCE GROUT (when extended with pea gravel) manufactured by Dayton Superior.
