AMENDMENT 1

17 August 2009

SECTION C – DESCRIPTION/SPECIFICATIONS/WORK STATEMENT

SCOPE

The purpose and intent of this contract is to replace the septic tank and if necessary install a new sewer line and subsurface disposal field at the Oak Flat Station. The Oak Flat Station is located on the Golden State Hwy about 3 miles north of the Templin Highway exit on Interstate 5.

Upon award of the Contract, the Contractor shall provide labor, materials, equipment, supervision, and incidentals necessary to perform the Statement of Work.

The Contractor shall assume all risk, loss, damage, or expense arising out of prosecution of the work, except as otherwise provided in the contract.

PROJECT DESCRIPTION

The Contractor shall locate the existing septic tank inlet and outlet piping. Remove, sanitize, and dispose of the existing septic tank and purchase and install a new 2000 gallon concrete septic tank. The Contractor shall then connect the inlet and outlet piping to the septic tank and video monitor the discharge piping for damage. The damaged piping shall be restored to the existing subsurface disposal field.

The Contractor shall then video monitor the existing disposal field and provide a copy of the video to the Contracting Officer for review. The Contracting Officer shall then determine if the existing disposal field is to be abandoned and a new subsurface disposal field constructed. The Decision will be made within 7 days of receipt by the Contracting officer of the video.

If the decision is to replace the existing subsurface disposal field, the Contractor shall install approximately 300 feet of 4 inch ABS drain line with 30 inches minimum cover, and new leach field with 18 inches minimum cover. The Contractor shall provide cleanouts at a minimum interval of 100 feet and at any change of direction or slope. Leachfield trench shall be 3 feet wide and 3 feet below perforated pipe. Trench to be filled with graded rock from 2 inches above the pipe to the bottom of the trench. Top of rock shall be covered with a fine mesh geotextile to prevent backfill from penetrating the rock fill. Total leachfield length to be 400 feet in four legs complete with a distribution box. Prior to beginning construction of the drain line and leach field, the Contractor shall survey the alignment for the 4 inch ABS drain line and the leachfield and provide a scale drawing of the alignment with a profile for approval by the Contracting Officer.

The Specifications and Drawings contained in the contract documents are included here by reference.

Directions: A vicinity map is included to show the location of the Oak Flat Fire Station. From Interstate 5 take the Templin Hwy exit and turn west to the Golden State Hwy. Turn Right and proceed north about 3 miles to the entrance to the Oak Flat Fire Station on the left.

To minimize the potential for spreading and/or introducing invasive plant species, the following precautionary measures will be followed:

1) All equipment except trucks, vans, pickups, and cars used for daily transport of personnel, will be cleaned prior to entering Forest Service land. This includes wheels, undercarriages and bumpers. All washing must take place where rinse water is collected and disposed of in either a sanitary sewer, a landfill, or other facility authorized to accept such rinse water.

2) Oak Flat is one of only a few southern California locations highly infested with yellow- star thistle. As a result of this localized infestation, equipment, tools and all vehicles will also need to be washed after leaving the work site when work is completed.

4) Holder shall certify in writing compliance with the terms of this provision prior to each start-up of operations.

5) Any new infestations of invasive species in the work area or on the access route shall be promptly reported to the Forest Botanist.

6) Upon request, the Forest Botanist can provide a current list of invasive species of concern.
