Amendment No. 2 Solicitation No. AG-9104-S-09-0003

Hollywood Reservoir-Lakeridge Drive Landslide and Slope Restoration Project.

Description of changes:

1. The date set for receipt of bids is changed to 7/30/09 at 3:00 p.m. local time.

2. The attached Bid Schedule Pages 13a and 13b replace Bid

Schedule page 13 shown in the solicitation.

3. Revise Clause H.19 as follows:

Lines, Grades, and Measurements: The Department has established primary survey control. The Contractor shall preserve all benchmarks, monuments, survey marks, and stakes installed by the Department. An amount of $500.00 will be deducted from any moneys due the Contractor for the replacement of each benchmark, monument, survey mark, and stake disturbed or removed by anyone other than the Department.

Using the Department’s supplied control, the Contractor shall set all additional lines, levels, targets, and batter boards required for the work and shall be responsible for their accuracy. The Engineer may on occasion check lines and levels set by the Contractor, but the responsibility of all dimensions shall rest entirely with the Contractor.

The Contractor shall notify the Engineer in writing 3 working days in advance for a request to provide survey control. The Contractor shall give the Department notice not less than 5 working days before disturbing any survey monuments.

The Contractor shall notify the Engineer 72 hours before the completion of site clearing and beginning excavation for each slide. The Department’s Survey crew will perform the initial survey for each slide. This initial survey will be used to determine the overall fill quantities for each slide for the work described in Bid Items 3 and 13. The Department’s Survey crew will require two days to conduct the initial survey for each slide area.

After inspection and approval of the completed excavation for the keyway and each individual bench and before any fill is placed on the approved foundation, the Department’s survey crew will map each keyway and or bench as the earthwork proceeds. This mapping of the completed excavation and the initial survey will be used to determine the overall fill quantities for each slide for the work described in Bid Item 13.

4. Add the following paragraph to Clause H.20:

All delivery personnel shall have Department supplied ID badges issued to them or shall be escorted through the gates to the construction site by a contractor employee with a Department issued ID badge.

5. Revise Clause H.28 as follows:

Construction Access: The Contractor shall limit all ingress and egress to the Hollywood Reservoir entrance off Lake Hollywood Drive as shown in the drawings.

This access is guarded by several locked gates. These gates shall remain closed and locked at all times. If the contractor is expecting delivery shipments and wants the gates to temporarily remain open, flagmen with a Department issued ID badge shall be posted at each gate. The contractor can install his own set of locks to expedite the unlocking of each gate by a contract employee.

A second access road off of Cahuenga Boulevard East will be available ONLY for the Contractor’s drill rig and crane.

6. Replace SECTION F02260 EXCAVATION SUPPORT AND PROTECTION, Section 3.01 F, with the following wording:

F. All required field welding shown on the approved soldier pile submittals shall be performed under continuous inspection by a LA City Deputy Welding Inspector.

7. Revise Specification F02300, Section 3.01 EXCAVATION as follows:

a) Delete Section 3.01.B. Salvaging and stockpiling of topsoil is not required.

b) Section 3.01.C. is revised as follows: All disturbed, loose, desiccated, cracked, slaked, or overly wet soils, and any other undesirable soil or material from the subgrade of the structures, shall be removed and replaced with suitable material. If the excavation of the keyway or benching is required beyond the limits on the drawings, the completed excavation will be measured and included in the measured pay quantity as described in Section 4.01 Measurement and Payment. If the aforementioned soil conditions are caused by improper construction activities or by failure to provide protection for the soils, including protection from wetting or drying by natural causes, the Contractor shall remove and replace the unsuitable material with suitable material at no additional cost to the Government.

8. Revise SECTION F02300 EARTHWORK, Section 4.01 as follows:

4.01 Measurement and Payment

A. At the end of each month, for each slide, the

Contractor shall submit computations, for the Engineer’s review, indicating the volume of fill.

1. For Bid Item 3, the pay limits will be the difference between the neat lines shown on the drawings for the finished grade and the initial survey of the site following site clearing described in Clause H.19.

2. For Bid Item 13, the pay limits will be the difference between the initial survey of the site following site clearing and the measured surface of the foundation when approved for placement of fill as described in Clause H.19.

3. Northern Slide, Middle Slide, and Southern

Slide.

B. Payment will be paid on a cubic yard basis at the contract unit price for Bid Item Number 3 and 13.

Such payment shall be considered full compensation for furnishing all labor, materials including imported borrow material, tools, equipment and doing all work including grading, shaping, disposing of the excess material, restoring adjacent ground to original condition and other work required under this Section or by the drawings.

9. Replace Drawing C-2.10 with the revised Drawing

C-2.10(R). The revised drawing includes sources of water for use for construction and the location of the concrete washout area.

10. Replace drawing C-5.00 with sheet C-5.00(R) showing the revised perforated pipe details.

11. Traffic and equipment loads on the LADWP private road and West Patrol road shall be limited to AASHTO H-20 loading.

12. Temporary access ramps to the base of the landslides may be constructed using temporary fill. The hillside shall not be cut in any areas not specified in the construction drawings without the permission of the LADWP and the Geotechnical Engineer of Record, Bob Hollingsworth.

Temporary stockpiles of cleared material and excavated material may also be placed on the West Patrol road and shown as a staging area on the drawings.

All temporary fill and stockpiles shall be removed at the end of the project. A 24-inch culvert pipe shall be placed beneath temporary fills and stockpiles along the roadway to allow for drainage. The grading for the temporary access ramps and stockpiles shall allow for vehicle access through the site by LADWP security patrols.

13. Contractors may request AutoCAD drawings from Ray Miller at 530-792-5679. However, the approved construction drawings in .pdf format shall be used to define the work.

14. Add Drawings, Access Road Plan.pdf and Access Road

Profile.pdf showing the plan and profile of the temporary access road from Cahuenga Blvd. for mobilization and demobilization of the drill rig and crane.

15. The attached Department of Labor Wage Determination dated 7/10/09 replaces the previous Wage Determination included as an attachment.

16. Revise Specification F02466, Section 1.02.A. to the following: Construct 24-36 inch diameter pilot holes.
