The government will reestablish horizontal and vertical control points missing before the beginning of construction. Using these as a reference, the Contractor shall establish adequate additional control points to accomplish the work. The Contractor shall protect, maintain and replace control points as necessary.
3. Traffic Control:
Traffic control devices for all construction activities, including full and partial road closure, shall be maintained for the duration of construction. The Contractor shall submit for approval by the CO, a traffic control plan prior to construction. The plan shall conform to the most recent edition of the Manual on Uniform Traffic Control
Devices. The Contractor shall provide and maintain all necessary signs. Temporary traffic control is indirectly paid by item 262(01).
4. Erosion Control:
Prior to the start of construction, submit a written Erosion Control Plan that provides permanent and temporary erosion control measures to minimize erosion and sedimentation during and after construction. Payment for the Erosion Control Plan is included in Pay Item 157(14). Do not begin work until the necessary controls for that particular phase of work have been implemented. Do not modify the type, size, or location of any control.
An alternate erosion control plan with all necessary permits may be submitted 5 days before intended use.
Incorporate all permanent erosion control features into the project at the earliest practical time, as outlined in the approved plan. When erosion control measures are not functioning as intended, immediately take corrective action. Payment for Erosion control is included in Pay Item 157(13).
Key features for the Erosion Control Plan inlude methods to prevent sediment from the excavation from reaching Whetstone Branch below the slide and methods to contain sediment from stockpile and disposal areas.
5. Temporary Stockpile Areas:
Roadway and suitable areas adjacent to the site may be utilized for temporary stockpiling of materials as approved by the CO.
6. Archeological Requirements:
In the event that an unknown historic or prehistoric site is discovered in the course of the project, the activity will be stopped, the CO notified, and the appropriate measures will be taken to stop any adverse effects to the site resulting from the activity. No further site disturbance is permitted without written approval of the CO.
8. Excavation and Embankment:
A: Any required excavation for access will be confined to the limits of the existing roadway. Excavated areas created for access shall be backfilled and compacted in accordance with FSSS Section 209, Compaction
Method B. All excavation and embankment is included as incidental or indirect payment to 262(01).
B: All material excavated and determined by the contracting officer to be suitable shall be preserved and used within the Reinforced Soil Slope.
C: Supplemental backfill (KY DGA or approved substitute meeting the requirements of FP-14 Section 704.03), shall be placed and backfilled in accordance with FSSS Section 209 Compaction Method B. Payment for supplemental backfill is by item 322(02).
D: Soil backfill shall be placed within a minimum of 1' from the finished slope facing to promote vegetation growth.
12. Corrugated Metal Pipe Installation
All corrugated metal pipes (CMPs) shall be Aluminized, Type 2 CMPs.
Class 2 riprap (FSSS 705.02) or larger for the energy dissipator at the 18" CMP outlet may be conserved from the excavation or shall be obtained from a commercial source and paid for by item 251(01).
Bedding material shall meet the requirements of FSSS 704.02. Payment for bedding material is incidental to
Item 602(01).
Culvert backfill material obtained from the excavation shall conform to the requirements of 704.03 where placed within 1' of the culvert. Otherwise, the material shall conform to the requirements of 704.06, and compacted in accordance with FSSS 209.09 and FSSS 209.10, Compaction Method B.
13. Road Reconstruction:
Ensure a consistent grade and smooth transition between reconstructed road segment and undisturbed road segment.
Road and ditch reconditioning and reconstruction shall be paid by item 303(01).
Aggregate surfacing shall be paid by item 322(01).
11. Installation of Erosion Control Mat:
Install Erosion Control Mat per Section 629, and in accordance with manufacturer's reccomendations.
See Sheet 10 for additional details and specifications regarding installation of Erosion Control Mat.
9. Construction of Reinforced Soil Slope:
Construct Reinforced Soil Slope per FSSS 262 and manufacturer's recommendations. See additional construction notes on Sheets 5, 6, and 9.
2. Notifications:
A: The Contractor shall notify the CO at least 10 days prior to the start work date.
7. Clearing and Excavation:
A: Clearing and grubbing of existing surfaces is required, indirectly paid by item 262(01).
B: All trees, brush, and debris shall be stockpiled separately and scattered on the fill slopes adjacent to the reinforced soil slope following completion.
C: Conserve, stockpile, and reuse all suitable excavation in the construction of the reinforced soil slope.
Excavation unsuitable for use as backfill shall be disposed of at a former quarry site, approximately 2 miles south of the project site, along State Highway 89, see Vicinity Map on Sheet 1. Dispose of material in locations designated by the Contracting Officer with slopes not to exceed 1V:2H. Material deemed unsuitable due to size may be used on site as riprap. Payment for material disposal is incidental to Pay
Item 262(01).
10. Geocomposite Sheet Drain System
