AMENDMENT NO. 1

SOLICITATION NUMBER RFQ AG-531A-S-16-0015

NFSR 43, WHETSTONE SLIDE REPAIR

ON THE LONDON RANGER DISTRICT

DANIEL BOONE NATIONAL FOREST

Pre-Bid Site Visit Notes The pre-bid site visit meeting was held as scheduled on September 7, 2016 at the project site. The Forest Service Representatives in attendance were Andrew Christensen and Chris Phelps. The following contractors were represented at the meeting:

1. Ground Solution

2. Jigsaw Enterprises

3. Siler Excavating

4. Grimes Construction Co., Inc.

5. E & J Construction Co., Inc.

6. Advanced Enterprises, Inc.

7. Diaz Construction, Inc.

Pre-Bid Meeting Agenda:

Andrew Christensen went through the pre-bid meeting agenda shown on Page 4 with those in attendance.

Questions and Answers:

The following questions were asked and recorded. The answers to the questions are addressed below:

(1) What are the disposal requirements for the fallen trees within the excavation?

a. It was stated that the trees could be decked alongside the road (outside of travelled way) and/or scattered on the fill slopes adjacent to the constructed reinforced slope.

b. Note #7 on Sheet 12 of the drawings clarifies disposal requirements.

(2) Will the contractor be required to gate or block the road during construction?

a. It was stated that yes, traffic control would be the responsibility of the contractor. A follow up question was would barricades be the minimum requirement, and it was stated that barricades and “road closed” signs would be the minimum.

b. Note #3 on Sheet 12 of the drawings specifies that the contractor is responsible for traffic control, and requires a traffic control plan prior to construction.

(3) What would be the proposed start date?

a. It was stated that we would like to start as soon as possible, to take advantage of the typically drier weather present during the fall. It was clarified that ideally, work would start by mid-October.

b. Page 10 of the solicitation (FAR 52.211-10) requires work to commence within 10 calendar days after issuance of Notice to Proceed.

(4) Clarification was requested regarding “construction limits,” “clearing limits,” “excavation limits,” and the “slide perimeter” that is shown on Sheets 3-4 of the drawings.

a. It was stated that the construction and clearing limits were the road template from Station 0+00 to Station 4+00 for road/ditch reconditioning, reconstruction, and culvert installation, in addition to the excavation limits for the reinforced soil slope.

b. It was stated that if additional trees needed to be cut, they would have to be identified and cleared by the COR prior to cutting.

c. It was stated that clearing and excavation was not required within the “slide perimeter” north of the road and reinforced soil slope limits. However, ditch reconstruction will be required through the slide perimeter. Any stabilization needed on the cut slope north of the road would be a changed or differing site condition.

d. The slide perimeter was displayed in the drawings to visually illustrate the slide location in reference to the roadway and reinforced soil slope. Portions of the slide perimeter are likely outside the excavation limits both above and below the reinforced soil slope.

e. Excavation limits are described in Note #3 on Sheets 5-6 of the drawings.
