The offeror shall present factual material dealing with contracts performed for other Government agencies or with private sector businesses for which the same or similar services were provided. When problems were encountered, briefly describe the problem areas and the steps taken to resolve or correct. For those contracts with less than positive performance, you are encouraged to provide a description of the problems and the efforts made to correct and to prevent future occurrences. Drum Chopping IDIQ, Conecuh Ranger District AG-4146-S-15-0006

(ii)The description of work performed should be in sufficient detail for the Government to determine what the work included. Completed questionnaires should be submitted no later than the offer due date and time indicated on Standard Form 1442. However, any PPQs may be accepted up until the evaluation starts.

(ii) If the contractor fails to submit PPQs, this could result in the contractor receiving a neutral rating for past performance. When only 1 to 2 PPQs are received in response to a solicitation, a lower past performance rating could result due to limited information available on which to base an evaluation.

(iii) If the prime contractor has limited past performance for the relevant work and a sub-contractor will be performing this work, then PPQs for the subcontractor need to be submitted –along with the sub’s work experience. (Limit to 1 sub and up to 5

PPQs) Clearly indicate the prime contractor for which the PPQ is being furnished. The prime contractor still needs to have PPQs submitted for evaluation purposes.

3) Offerors lacking recent and relevant present/past performance experience may submit information regarding key personnel who have recent and relevant experience. In this case, offerors shall provide the following information, as a minimum, to allow government evaluators to conduct a present/past performance evaluation for those key personnel cited by the offeror.

(iii) Telephone Number

(iv) Brief synopsis of relevant experience (a resume may be submitted)

The experience information should not to exceed more than 25 pages (8 ½ x 11).

(3) The experience questionnaire (Attachment 3) can be completed to address the experience and past performance requirements.

Additional information may be submitted to supplement this form as necessary. Offerors need to ensure all necessary past performance information is provided as noted above.

NOTE: “Relevant past performance” is defined as drum chopping or forestry heavy equipment operations for the same or similar to those in the specifications contained in this solicitation.

FAR 52.212-2 Evaluation – Commercial Items (Oct 2014)

Drum Chopping IDIQ, Conecuh Ranger District AG-4146-S-15-0006

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.

Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

ADDENDA to 52.212-2 EVALUATION –COMMERCIAL ITEMS (Oct 2014)

(d) BASIS FOR CONTRACT AWARD: The basis for award will be an integrated assessment based on tradeoff between past performance, experience, and price. The Government will award the contract to the offeror who represents the best value to the

(2) Reasonableness: Prices are reasonable in comparison to estimates, prior prices paid, and competition, and can be supported by suitable estimating techniques. The Government reserves the right to perform a price realism analysis to determine whether an offeror’s proposed prices are realistic for the work to be performed. If a price is determined unreasonably low to realistically perform the work for the proposed price; the offer will not be considered for contract award.

2. PAST PERFORMANCE: Past performance assessments shall be accomplished for all offerors. Each offeror shall be assigned one of the following ratings:

RATING DEFINITION

(2) An offeror with limited or no recent or relevant past performance will receive a rating of

Neutral/Unknown Confidence, and will not be rated favorably or unfavorably on past performance.

(3) Past performance shall be evaluated on the basis of the following sub factors of equal importance:

i. Contract Performance in projects of the same size and scope ii Timeliness of Performance

. iii. Quality of Service/Control

3. EXPERIENCE: The offeror’s experience shall be evaluated on the basis of if the offeror meets or exceeds the desired qualifications specified below and has shown the experience and skills required to perform the tasks similar to those identified in this solicitation. Experience will be evaluated for only those offerors in the competitive range. (Expanded as needed)

Desired Qualifications:

The offeror has experience with at least three drum chopping projects or forestry heavy equipment operations of the same size and scope of this project within the past 5 years.

Offerors with drum chopping experience will be rated higher than an offeror with forestry heavy equipment operations.

A project is work completed within a specified period of time. For work done on continuing annual basis, a project is the work completed in any given year.

4. Past Performance and experience when combined is more important than price.

5. The Government reserves the right to award the contract to other than the lowest priced offeror.

(e) AWARD WITHOUT DISCUSSIONS. The Contracting Officer will only clarify the offerors’ information; no discussions are contemplated in accordance with FAR 15.306. Offerors may be given the opportunity to clarify relevance or adverse past performance to which the offeror has not previously had an opportunity to respond. Clarifications are limited exchanges between the

Government and offerors that may occur when award without discussions is contemplated. If discussions are determined to be necessary the procedures described in FAR 15.306 will be used.
