LETTER OF INTRODUCTION
SNAP STORE VISIT CONTRACT
United States Department of Agriculture Food and Nutrition Service
Benefit Redemption Division
Letter of Introduction
3101 Park Center Dr Alexandria, VA 22302-1500
The United States Department of Agriculture’s Food and Nutrition Service (FNS) has hired contracted review staff from xxxxxxxxx to visit stores. The purpose of the visit is to collect and verify store information needed by FNS staff to determine eligibility or continued eligibility to participate in the Supplemental Nutrition Assistance Program (SNAP), formerly known as the Food Stamp Program.
In order to conduct a review of this store, review staff will take notes on store inventory, make a sketch of the store layout, and take photographs inside and outside of your store.
This will take about 30 minutes and should not interfere with normal store operations.
Reviewers have no authority to answer SNAP questions or make comments or decisions regarding a store’s eligibility to participate in the program.
FNS cannot allow a store to accept SNAP EBT cards without a complete store review.
SNAP rules require retailers to provide FNS with necessary information when requested.
If you do not allow a store review to be conducted, your application may be denied or your permit may be withdrawn.
If you agree to allow this reviewer to conduct a review of your store, please sign the Store Review Consent Form. If you do not agree to allow this review, please sign the Store Review Refusal Form.
Keep this Letter of Introduction for your records. If you have any questions, please contact the SNAP Retailer Service Center at 877-823-4369.
Thank you for your interest in serving SNAP customers in your community.
