About this file

The document is an Affirmation and Disclosure Form related to a state contract opportunity that requires contractors to affirm and disclose details about their business operations, service locations, and data storage. The form mandates that contractors confirm their understanding of Executive Orders prohibiting offshore services, locating state data offshore, or purchasing from Russian institutions or companies. Contractors must provide comprehensive information about their principal business location, service performance locations, and state data storage sites, including details for both the primary contractor and any subcontractors.

The form emphasizes ongoing transparency, requiring contractors to immediately disclose any changes in service or data location before, during, and after contract execution. The state retains the right to immediately terminate the contract if services are performed outside the United States without a signed waiver. The document serves as a critical compliance mechanism, ensuring that contractors operate within specified geographical and operational constraints, with potential contract termination as a consequence for non-compliance. The form must be signed by an authorized representative of the contractor, acknowledging their understanding and agreement to the specified terms.
