About this file
This is an Affirmation and Disclosure Form (Rev. 3/22) that must be completed by contractors entering into contracts with the State. The form requires contractors to affirm their understanding of Executive Orders prohibiting offshore services, offshore data storage, and purchasing from Russian institutions or companies. The form requires detailed disclosure of business locations and service delivery locations for both the primary contractor and any subcontractors.
The form mandates disclosure of three key categories of information: principal business locations, locations where services will be performed, and locations where state data will be stored - for both the contractor and all subcontractors. The contractor must notify the State of any changes in service locations, with the State retaining the right to immediately terminate contracts if services are moved outside the United States without an approved waiver. The form must be signed by an authorized representative of the contractor and becomes incorporated into any contract with the State.
