About this file
This is a Required Affidavit for Claiming Resident Bidder Status issued by the Commonwealth of Kentucky for use in state and local contract bidding. The affidavit applies to individuals, partnerships, associations, corporations, and other business entities seeking to claim resident bidder status in accordance with Kentucky Revised Statutes (KRS) 45A.494(2). Bidders and offerors must swear under penalty of perjury that on the date the contract is first advertised, they are authorized to transact business in Kentucky and have maintained compliance with three specific requirements for the one-year period prior to and through the date of advertisement: filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund as established in KRS 341.49, and maintained an active Kentucky workers' compensation policy.
The bidding agency reserves the right to request documentation from any bidder claiming resident status to verify compliance with these requirements. Failure to provide requested documentation upon demand will result in bidder disqualification or contract termination. The affidavit requires the bidder's signature, printed name, title, and date, along with company name and address. The document must be notarized, with the notary public providing their seal and commission expiration date. This affidavit serves as a compliance verification tool for the procurement process related to the Flex Insight Workstations contract opportunity in Kentucky.
