About this file

This is an Affidavit of Eligibility Form for the Opportunity Zone Program, Version 12.25, applicable to Florida state and local contract opportunities. The form serves as a certification document for vendors and contractors seeking to participate in projects within designated Opportunity Zones. The affidavit establishes the eligibility criteria and requirements that prospective bidders must meet to qualify for consideration on state and local contracts, including the HVAC Chiller Overhauls project at CHOT and NSCEP locations. The document requires applicants to verify their business status, ownership structure, and compliance with program-specific regulations before submitting proposals or bids for these opportunities.

The affidavit form establishes baseline eligibility standards for participation in Florida's Opportunity Zone Program and related contracting initiatives. Vendors and contractors must complete and submit the affidavit as part of their proposal packages, affirming their qualifications and adherence to program requirements. The form functions as a pre-qualification mechanism to streamline the bidding process and ensure that only eligible entities proceed with formal bid submissions. Completion and submission of this affidavit are mandatory for all interested parties seeking to bid on state and local contracts within the designated program framework.
