Statement of Work

GENERAL REQUIREMENTS

Contractor will provide all supervision, labor, equipment, materials, and transportation to clean out, transfer, and prepare all Aqueous Fire Fighting Foam (AFFF) concentrate and resultant mixtures from a Coast Guard-owned 2013 Oshkosh Striker 1500 Aviation Rescue Firefighting

(ARFF) truck for disposal. In order to appropriately dispose of approximately 415 gallons of

AFFF concentrate and associated mixtures and rinsates generated from remediation work in accordance with applicable laws and regulations, it must be transferred to appropriate containers and transported for final disposal off-island. Base Kodiak must consolidate and dispose of all

AFFF concentrates, mixtures, and contaminated media in alignment with DOD policy and directives.

This scope of work overviews the major items of work and in no way limits the details and work the contractor is responsible for that is not included in this document. This may include requirements in referenced documents and code requirements. The contractor is responsible for a high level of workmanship in the execution of work.

U.S. Coast Guard (USCG) Base Kodiak requires services for the preparation, shipment, and off-site disposal or recycling of non-regulated and universal waste and/or other wastes.

Contractor will provide all supervision, labor, equipment, materials, and transportation for the cleanout, transport, and disposal of the contents of a truck.

SCOPE

The following items comprise the scope of work for this project. Items will not necessarily be completed in the order listed.

1. Execute rinse and cleaning methodology to flush AFFF from CG-owned ARFF truck.

1.1. Truck tank, piping, and fittings must be flushed and rinsed thoroughly to include a full rinse with a combination of water, neutralizing, or binding agent such that final rinsate generated achieves the lower of the two following metrics:

1.1.1. Non-detect, or below lab reporting levels for PFOA and PFOS

1.1.2. Below EPA MCLs of 4 ppt for PFOA and PFOS

1.2. Sample rinsate generated after each flushing evolution of truck tank, piping, proportioning system, pump, discharges, foam tank drains, system relief valves, roof and bumper turret, 2 side hose discharges, and booster reel as well as any other foam concentrate or foam contaminated components of the truck.

1.2.1. All product and rinsate generated must be captured and contained.

1.3. Stow all contaminated media and PPE created by contractor operations directly covered in the requirements of this contract into waste accumulation containers. Label containers in accordance with Attachment A.

2. Weigh and label IBC totes and waste accumulation containers with the weight and the information specified in Attachment A.

2.1. At the Government's option, the use of USG scales will be allowed. When onsite

USG scales are available, weights must be agreed upon by both the Contractor and the USG before removal. When offsite commercial scales must be used, a certified weight ticket will be provided to the COR as stated above and shall be the basis for payment to the Contractor.

3. Characterize, sample, and transport waste off Base Kodiak premises and to an appropriate final receiving facility which will properly dispose of the product, contaminated media, and rinsate IAW all local, state, and federal laws and regulations.

Deviation from the tasks requires government approval. Unsatisfactory performance shall result in work being re-accomplished at no cost to the government.

The Contractor shall keep the COR fully informed on the daily progress of the work, from the start through completion of this project. The government shall use a 100% surveillance method.

Inspection of work shall be performed periodically while work is being performed and prior to contractor leaving the site. The COR will conduct these inspections and provide final acceptance of the work.

APPLICABLE DOCUMENTS

Attachment A: USCG Base Kodiak Regulated Waste Container Management

PERFORMANCE PERIOD

Work will be completed within 30 days after Date of Award.

WORK HOURS

Normal working hours for this project shall be Monday through Friday 0730-1630 excluding all

Federal holidays. Work outside these hours shall require verbal or written approval of the

Contracting Officer or his/her authorized representative.

WORK RESTRICTIONS

In no event shall any understanding or agreement between the contractor and any government employee other than the Contracting Officer on any contract, modification, change order, letter or verbal direction to the Contractor be effective or binding upon the Government. Formalize all such actions by a proper contractual document executed by an appointed Contracting Officer. The contractor is hereby put on notice that in the event a government employee, other than the

Contracting Officer, directs a change in the work to be performed, or increases the scope of the work to be performed, it is the contractor's responsibility to make an inquiry to the Contracting

Officer before making the deviation. No payments will be paid without being authorized by an appointed Contracting Officer with the legal authority to bind the government.

Contractor access and use of premises: Ensure that contractor personnel employed on the base become familiar with and obey base regulations. Personnel shall be appropriately dressed for the work and weather and materials they are in contact with. Wear hard hats and protective equipment in designated areas. Do not enter any restricted areas unless required to do so and until cleared for such entry. The contractor’s equipment and personnel shall be conspicuously marked for identification.

Without additional expense to the USG, the Contractor shall be responsible for paying all fees, preparing, or obtaining any necessary licenses, permits, notifications, waste profiles, or reports, which result from a Contractor’s transportation, recycling, or disposal decision. This

20 November 2025 Page 3 | 5 includes any fees associated with requirements of the Environmental Protection Agency

(EPA) national electronic manifest (e-Manifest) system. This includes acquiring any required permits or registrations necessary to operate on any installation listed in this contract and completing the Notification of Regulated Waste Activity form for the Generator(s) of hazardous waste to sign and file with the applicable state regulators for waste disposal.

The work area must be free of hazards, materials, and equipment during non-working hours. The contractor will erect barricades closing the work area off to traffic as required and shall erect barricades closing the work area off to pedestrian traffic during work performance. Barricades shall be removed before the end of each work period unless the work requires barriers to remain overnight for safety.

The Contractor and their Subcontractors must always perform in a prudent, conscientious, safe, and professional manner. The Contractor shall ensure that its agents, employees, and

Subcontractors involved in handling and packaging the hazardous waste are trained for the level of expertise required for the proper performance of the task; in particular, the areas of chemical incompatibility, general first aid procedures, and spills. The Contractor shall provide all Materials Handling Equipment (MHE) and Personal Protective Equipment (PPE), unless otherwise stated. The MHE and PPE must be appropriate to safely handle hazardous waste, to include meeting any additional military installation specific requirements.

If road closures are necessary, provide alternate traffic plan to USCG Base Facilities Engineering, Fire Department, and Military Police. If road or parking lot closures are necessary, provide alternate traffic plan to USCG Base Facilities Engineering, Fire Department, and Military Police.

PERMITS/NOTIFICATIONS

Contractor shall notify the COR in the event of an outage of any facility.

Coordinate with Base Kodiak Environmental and HazMat personnel for access to required sites as well as Base Kodiak Fire Department personnel for access and operations that may be conducted on Fire Department grounds. The required access keys shall be checked out and returned daily with the Base Kodiak Military Police.

SUBMITTALS REQUIREMENTS

Certificates of Disposal/Destruction (CD) or Recycling for any/all items are required. Some facilities issue CDs or Certificates of Recycling that, in essence, state the waste “will be” versus “has been” recycled, disposed, or destroyed IAW applicable regulations. Such certificates will be authorized under this Section, but only when issued and certified by the final recycling or disposal facility. CDs and Certificates of Recycling shall be provided to the

Generator and the COR within 30 days after disposal, destruction, or recycling occurs. The

Contractor will not be authorized payment until all required Certificates of

Disposal/Destruction (CD) or Recycling are received by the COR and Generator.

RETURN OF SHIPPING DOCUMENTATION

At the time of removal, the Contractor shall provide the Generator/USG Representative with a complete and accurate, transporter-signed copy of each manifest that is used for the waste removal. If the Generator/USG Representative is not present at the time of removal, the

Contractor shall provide the COR with a copy of the manifest(s).

The use of transfer facilities is authorized during transit of waste to the designated facility. A transfer facility is any transportation related facility including loading docks, parking areas, storage areas and other similar areas where shipments of hazardous waste are held during the normal course of transportation, per 40 CFR § 260.10. The Contractor will ensure that the 10-day holding time is not exceeded at any transfer facility.

If within 30 days from the removal date, the waste has not yet been received at the designated

TSDF, the Contractor shall notify the COR of the current physical location of the waste.

Within 45 days from the removal date, the Contractor shall provide the COR with a facility-signed electronic copy of each manifest via email. If multiple transporters were used during transit, the Contractor shall also provide the COR with a Uniform Hazardous or Non-

Hazardous Waste Manifest Continuation Sheet, as applicable, with additional transporter signatures as necessary.

If the Contractor is unable to deliver the waste to the designated or alternate facility identified on the manifest(s) per 40 CFR § 263.21(a), the COR and Generator will be contacted for disposition and manifest revision instructions per 40 CFR § 263.21(b). The Contractor shall make reasonable attempts to deliver the waste to a qualified alternate facility rather than returning the waste to the Generator. In the event of rejected shipments, or where the shipment is undeliverable, the Contractor shall provide the Generator with a written narrative of the incident, including details of all attempts to provide delivery to a qualified TSDF. If the manifest copy cannot be provided within 45 days from the date of removal, the

Contractor shall notify the generator and COR electronically with the current location of the manifested materials and the expected date of return for the manifests.

CLEANUP

Remove rubbish and debris from the project site daily. No materials may be stored at the work area unless approved by the Contracting Officer.

The Contractor is responsible for proper disposal of regulated waste. On-base dumpsters and waste receptacles on the fire department grounds will not be used to dispose of generated regulated waste.

Attachment A: September 2024 – USCG Base Kodiak Regulated Waste Container

Management

Containers used for accumulating RW must be:

1. New or certified reconditioned containers are to be used. Reconditioned containers must be clearly marked with the DOT recondition Identification

Number.

2. In good condition that means it is not leaking, rusted (i.e., more than minor surface rust), corroded, dented more than 2 inches, or have non-working filler caps/bungs and/or other sealing devices. It must not have any bulges, grooves

(other than removed metal), dents in seams/corrugations, or be deteriorated in any other way. If a container leaks or is not in good condition, the contents must be transferred to another container that meets all the standards or be placed in an over-pack.

3. Made of a compatible material that will not react or deteriorate when in contact with the material or waste.

4. Securely closed with RW cover or lid and must close and seal tightly enough to prevent spills or the release of fumes.

5. Storing wastes in open buckets is prohibited.

6. Containers must be tightly closed after every use.

7. Open-head containers must not be used for any liquids, unless pre-approved by

Base Kodiak Environmental Division.

8. In compliance with DOT-specified packaging (UN performance Oriented

Packaging) requirements.

Management:

1. All containers must be marked and labeled and placed in a temporary waste accumulation area, where they must remain until transported to N-48 for disposal.

2. All containers will be marked with Waste Description, Generator Information

(Company name, POC, Contact info, Accumulation start date if applicable) and

Contract/Task order.

3. Containers will be positioned in such a manner so that labels are easily visible.

Containers should not have to be moved in order to read labels.

4. Containers will be inspected weekly for leaks, corrosion, rust, and open lids/bungs.

5. A container will not be opened, handled, or stored in a manner that may rupture the container or cause it to leak.
