This document is an Addendum No. 04 with Written Inquiry Responses for a Request for Proposal (RFP) issued by the Florida Department of Children and Families (DCF), Office of Child and Family Well-Being, for the Child Abuse And Prevention Treatment Act (CAPTA) - Plans of Safe Care. The RFP seeks a Respondent to expand on existing early intervention and home visitation services for at-risk pregnant women, parents/caregivers using substances, and infants affected by substances and their families. The Vendor shall implement an evidence-based home visiting model to improve positive child development and behavior, and create individualized Plans of Safe Care for substance-affected newborns and their families. The budget should be based on available funding projections, if any. The RFP allows for an administrative rate of no more than 10% and includes specific performance measures, with financial consequences for not meeting minimum service targets.
