Addendum to 52.212-1 Instructions to Offerors Offerors are to submit any questions regarding this requirement via email to Evelyn.walker@va.gov on or before 1300 EST 11 MAY 2022 with the subject line, “RFQ – SUICIDE PREVENTION TECHNICAL ASSISTANCE AND EDUCATION DEVELOPMENT.” An amendment responding to all questions, which may or not extend the response due date, will be published at the GPE.
Offerors shall e-mail complete quotes to Evelyn.walker@va.gov at the date and time stated on the Combined Synopsis/Solicitation Notice and Description.
Provide the following information:
1. Solicitation number
2. Name and address of your company
3. Point of contact; name, e-mail and phone number
4. DUNS/UIN number
5. Unit and Total Prices as specified in Section B.2 Price/Cost Schedule.
6. A legible copy of current state licenses and offeror insurance coverage.
Note: To be considered for award, an offeror shall have an active registration in the System for Award Management (SAM).
