About this file

This is an Addendum 2, Version 3 document issued by the State of Minnesota Department of Military Affairs, Facilities Management Office at Camp Ripley in Little Falls, Minnesota. The addendum revises insurance requirements for a one-time contract to remove and recycle approximately 1,400 twin-size mattresses from Camp Ripley. The contractor will provide supervision and transportation to collect the mattresses and haul them to a recycling facility capable of either completely dismantling the mattresses into core components for transformation into new materials or disassembling them for complete refurbishment, with the requirement that mattresses not be redistributed or reused in their current form. Camp Ripley personnel will load the mattresses into the contractor's transport unit, which should be large enough to haul at least 200 twin-size mattresses per load, with the contractor responsible for transportation, delivery, and potential unloading at the designated recycling facility. The solicitation due date is May 22, 2026, no later than 1:00 p.m., with the buyer identified as Ms. Sarah Posterick of the Facilities Management Office.

The addendum modifies Exhibit B of the Sample Contract to establish comprehensive insurance requirements previously absent from the solicitation. Contractors must maintain Commercial General Liability Insurance with minimum limits of $2,000,000 per occurrence and $2,000,000 annual aggregate, Commercial Automobile Liability Insurance with $2,000,000 combined single limit, and Workers' Compensation Insurance meeting statutory requirements with minimum limits of $100,000 per employee and $500,000 aggregate. Additionally, contractors must carry Professional Liability and Errors and Omissions insurance with $2,000,000 per claim and annual aggregate limits, with deductibles not exceeding $50,000 without written State approval. Garagekeepers Liability or Property of Others insurance is required if warranty or service work is performed. All insurance companies must maintain an AM Best rating of A- and Financial Size Category VII or better, be authorized to do business in Minnesota, or be domiciled in Minnesota with appropriate Department of Commerce certification. The State of Minnesota must be named as an Additional Insured, and contractors must submit Certificates of Insurance prior to commencing work, with failure to comply resulting in immediate contract termination.
