About this file

This document is an Addendum (No. 1) for a Campus-Wide Roof Replacement project at Western State Hospital in Hopkinsville, Kentucky, issued by the Commonwealth of Kentucky's Department for Facilities and Support Services, Division of Engineering and Contract Administration. The addendum, dated June 10, 2025, is part of Request for Bid No. RFB-258-25 and introduces a key modification to the project's special conditions: all workers must now provide negative tuberculosis skin test results to work on-site. The addendum also indicates a change to the project drawings, specifically noting the omission of Sheet SP and its replacement with an attached Addendum #1 Sheet SP.

The pre-bid meeting sign-in sheet reveals seven attendees from various organizations, including representatives from PDM Architects, DECA (Division of Engineering and Contract Administration), CHFS (Cabinet for Health and Family Services), Kalkreuth Roofing & Sheet Metal, Barrier Roofs, and Galt Construction. The site plan and general notes provide extensive details about site utilization, construction requirements, safety protocols, and specific instructions for roof replacement, including debris removal, protection of existing structures, and maintenance of site security. The project involves roof replacements for multiple buildings (Buildings 20, 31, 33, 48, and 78) at the Western State Hospital campus, with comprehensive guidelines for contractor conduct, site preparation, and restoration.
