About this file
This is an addendum to an Invitation to Bid (RFP-58-27) issued by the Commonwealth of Kentucky Department for Facilities and Support Services, Division of Engineering and Contract Administration for the Kentucky Abandoned Storage Tank and Orphan Well Reclamation Program (KASTOW) Ohio 005. Addendum No. 1, dated September 4, 2026, revises the plugging activities requirements and provides detailed technical specifications for well abandonment work in Western Kentucky counties. The addendum specifies that plugging activities must be performed using a service rig and work string of steel tubing rather than slickline methods. The document establishes differentiated procedures based on well depth: wells with measured depth at 1,500 feet or shallower follow one protocol, while deeper wells require an alternative sequence of operations including staged cement plugging, perforation work, and enhanced isolation measures for protection of freshwater and coal seams.
The revised Attachment K establishes comprehensive step-by-step procedures for both well categories, requiring contractors to coordinate with Division inspectors, secure landowner access, construct access infrastructure including roads and lined pits, and perform specialized downhole operations involving tubing runs, cement placement, and perforation work. For wells deeper than 1,500 feet, the procedures mandate bond logs if top-of-cement records are unavailable, multi-stage cement plugging from formation-specific depths to surface, and casing removal or perforation from 250 feet to 50 feet in 50-foot increments. All work requires use of Class A, Class L, or Type 1L cement, with 50/50 POZ or latex-laden cement required in the presence of hydrogen sulfide gas. The addendum stipulates that all casing must be cut three feet below surface following cement verification and requires complete site reclamation including fluid removal, pit backfill, grading, and stabilization.

*PLUGGING ACTIVITIES MUST BE PERFORMED WITH A SERVICE RIG AND WORK
STRING OF STEEL TUBING TO RUN DOWNHOLE. “SLICKLINE” PLUGGING WILL
NOT BE ACCEPTABLE.
WESTERN KY (Ballard, Butler, Caldwell, Calloway, Carlisle, Christian, Crittenden, Daviess, Fulton, Graves, Hancock, Henderson, Hickman, Hopkins, Livingston, Logan, Lyon, Marshall, McCracken, McLean, Muhlenberg, Ohio, Todd, Trigg, Union, Webster)
Wells with TD at 1500’ or shallower
• Confer with Division Inspector, assure landowner access is approved
• Construct access road to well
• Dig pit and line with 6 mil liner
• Remove brush from location, rig-up service rig
• Establish wellhead entry if not open casing, or dismantle wellhead if fully equipped
• If tubing & rods remain in well, extract same from the hole and trip out
• Trip in well with rig tools to establish TD and ensure no wellbore obstructions
• Run tubing to TD and pump minimum five barrels of water or 6% gel
• If casing is not cemented or unknown; perforate casing from 250’ to 50’; (50’ increments)
• Circulate Class A, Class L, or Type 1L cement solid to surface (Cement calculation based on hole volume). 50/50 POZ or latex laden cement shall be used if H2S gas is present and/or detected.
• Monitor cement level and top off as needed
• Cut off all casing 3 ft. below surface the next day to ensure no fall back has occurred
• Remove fluids from pit, properly dispose of same, and backfill pit
• Properly grade, stabilize, and fully reclaim wellsite as outlined in Scope of Work
Wells deeper than 1500’
• Confer with Division Inspector, assure landowner access is approved
• Construct access road to well
• Dig pit and line with 6 mil liner
• Remove brush from location, rig-up service rig
• Pull Rods & tubing, if present
• Any free casing (annulus not cemented) shall be removed. If no records are available to determine Top of Cement (TOC), a bond log may be required
• Trip in well with rig tools to establish TD and ensure no wellbore obstructions
• Run tubing, spot 200’ Class A, Class L, or Type 1L cement plug at TD. 50/50 POZ or latex laden cement shall be used if H2S gas is present and/or detected.
• Pull up above TOC, wash out, Tag Bottom Plug
• Pump spacer water or 6% gel
• Spot Class A or Class L cement plug 50’ below deepest perforation to 50’ above shallowest perforation (or production zones if open hole), across any and all perforated (or production zones in open hole) not covered by bottom plug. 50/50 POZ or latex laden cement shall be used if H2S gas is present and/or detected.
• Pump spacer water or 6% gel
• If casing is not cemented or unknown; perforate casing from 250’ to 50’; (50’ increments)
• Spot Class A, Class L, or Type 1L cement from 600’ to surface (to isolate and protect fresh water and coal seams).
