About this file
This document is an Addendum No. 2 for an Invitation to Bid (ITB) on a Water Heater Replacement project issued by the City of College Park, Georgia. The project requires the replacement of water heaters, and the addendum modifies the original bid documents. It addresses specific questions raised by a single-member LLC bidder regarding worker's compensation insurance, auto insurance written under the owner's personal name, and the requirement for a 100% performance and payment bond. The addendum clarifies that the City only requires a bid bond, not the 100% performance and payment bond mentioned in the original bid documents.
The document also indicates that the bid number is 030524 and the bid proposal form is for the City of College Park. The project is scheduled to take place on February 27, 2024, and the bid package should include the form WC-10 regarding worker's compensation insurance. Additionally, the City specifies that all insurance should be identified under the business name when submitting a proposal, and a personal auto insurance policy under the owner's name will not be accepted.

ADDENDUM No. 2
OWNER’S BID PROPOSAL FORM
OWNER: City of College Park 3667 Main Street College Park, GA 30337
PROJECT: Water Heater Replacement
DATE: February 27, 2024
BID #030524 is hereby revised as instructed throughout attached ADDENDUM No. 2.
This Addendum forms a part of the contract documents and modifies the original bid documents.
The following items shall take precedence over the originally published bid documents for the above-named project and shall become a part of the contract documents.
1. I am a single member LLC, and therefore am not required by the state of Georgia to have Worker’s Compensation. Is there a waiver or any particular document I need to include in my bid regarding this?
a. Please include your form WC-10 in your bid package.
2. Being that I am a small business and a single member LLC, my auto insurance is written on a policy under my personal name as opposed to “business auto policy.” Before I proceed with this bid, will the city accept my auto insurance being a personal policy under my name?
a. All insurance should be identified under the business name, when submitting a proposal.
3. Page 15 states in parentheses that the 100 performance and payment bond is for construction and road projects. For clarification, is the 100% per performance payment bond required for the water heater project?
a. The City only needs the bid bond included in the proposal, we will not require the 100% performance and payment bond.
