About this file

This is Addendum No. 3 issued by the California Department of Transportation (Caltrans) Division of Procurement and Contracts for RFP 95A0082 - Traffic Safety Campaign. The addendum addresses three questions submitted by prospective bidders regarding the DOT ADM/1511 Subcontracting Provisions List form requirements. The questions and responses clarify that all subcontractors must be listed on the form regardless of whether they are being used to meet SBE or DBE requirements, and that all columns of the form must be completed in full, including contractor name, address, and description of work.

The addendum confirms that all other terms and conditions in the original RFP remain unchanged. The document is signed by Roberto Ramirez, Contracts Analyst, and dated December 12, 2024. The addendum is issued under Caltrans' mission to "provide a safe, sustainable, integrated and efficient transportation system to enhance California's economy and livability."

ADDENDUM No. 3 Request for Proposal (RFP) 95A0082

Traffic Safety Campaign

NOTICE TO ALL PROSPECTIVE BIDDERS:

Addendum No. 3 is being issued for the above-referenced RFP. Proposals for this work shall be submitted with the full understanding of this Addendum.

The following represents questions submitted for this RFP and the Department’s responses:

Question 1:

Can you please provide information on what needs to be included on DOT ADM/1511 Subcontracting Provisions List?

Answer 1:

Please refer to the instructions on the top of the form.

Question 2:

If we have subcontractors, but none of them are being used to meet SBE or DBE requirements, do we still fill out this form?

Answer 2:

Yes, please read the instructions on the form. It states, “List all Subcontractors that will be used in this Agreement.”.

Question 3:

Do we just list their name and their address, and the description of work that will be provided, but then we don’t need to submit a “quote” for them, since they aren’t being used to fulfill any requirements?

Answer 3:

Yes, as stated in the form instructions, “List all Subcontractors that will be used in this Agreement.” Please complete all columns of this form in its entirety.
