About this file
This is Addendum 2 to a Request for Proposals issued by Walworth County, Wisconsin for a Near-Site Clinic service. The addendum addresses questions submitted by vendors regarding the scope and operational requirements of the proposed clinic. The near-site clinic is intended to provide convenient primary care and acute healthcare services, improve early diagnosis and timely treatment, and offer coaching on healthier lifestyles to county employees. The service will operate on an outpatient basis with hours to be recommended by the responding vendor based on best practices for similar clinics. The clinic will serve both employees and spouses of Walworth County, with the patient volume and adult-to-child/adolescent ratio to be determined by initial utilization. Biometric screening events will occur at the end of May and beginning of June annually, conducted through venipuncture testing. Participants may submit laboratory results from their primary care providers as an alternative to onsite testing. The RFP deadline was 1:00 p.m. on March 15, 2018. Based on 2017 data, 682 employees and 394 spouses participated in biometric screening, with 1,019 tested onsite and 57 submitting offsite or physician-provided results.
Vendors should make recommendations regarding the clinical provider type—Nurse Practitioner, Physician Assistant, or MD—based on the RFP's service specifications and pricing model. The county does not seek to replace its current wellness program with Interactive Health but desires integration of coaching services. The county's current incentive structure offers employees and spouses with Interactive Health scores of 25 or below a minimum premium contribution discount (7% in 2017), while those with scores above 25 may participate in coaching sessions by October 6 to receive the same discount, with failure to complete coaching resulting in a middle-level premium contribution discount (10% in 2017). Proposals should be submitted on a jump drive or flash drive. Questions and clarifications regarding the RFP should be directed to Nick Cramer, Purchasing Manager, at Walworth County Public Works Department.
