About this file
This document is an addendum to a Request for Quotation (RFQ) issued by the City of Hartwell for lead-based paint testing and inspection services. The RFQ clarifies that the structures to be inspected may be occupied or vacant, and that while laboratory services are not initially required, if needed, they must be conducted by licensed and certified laboratories. The RFQ also provides response timelines, stating that lead-based paint testing and inspection services shall be completed within 20 business days after notification, written and visual assessment reports within 10 business days following the inspection, and clearance exams within 2 business days following completion of testing. Vendors may provide more than the minimum of 5 client references, and may submit resumes and project sheets as appendices, although these are not required.
The RFQ requires contractors and their subcontractors to submit affidavits with their proposals, and clarifies that the "class of work" refers to the type of work each subcontractor will perform. The RFQ also notes that additional tests may be added or deleted at the City's option, although this section was included in error and will be stricken.

December 15, 2023
ADDENDUM #1
REQUEST FOR QUOTATION RFQ No. 2023-1120
LEAD-BASED PAINT TESTING & INSPECTION SERVICES
Vendors are informed that the above subject solicitation is hereby modified, corrected, or supplemented as specified, described and set forth in this Addendum:
QUESTIONS/ANSWERS
Question 1: Should we anticipate that the structures to be inspected will be occupied or vacant at the time of inspection?
Answer 1: The structures will be residential properties and may be either/or.
Question 2: Under Section J (Additions/Deletions of Facilities) on page 2, the RFQ states
"Although this solicitation and resultant contract identifies specific lab test to be performed, it is hereby agreed and understood that additional tests may be added or deleted to/from at the option of the City" [emphasis added]. Please clarify which specific lab tests have been identified and are to be performed.
Answer 2: Section J of the RFQ solicitation should be stricken. It was left in the RFQ document in error.
Question 3: Under Section L (Insurance) on page 3, the RFQ states that "the bidder may submit a Certificate of Insurance with the quotation." Please confirm that a sample Certificate of Insurance will suffice.
Answer 3: Vendors may submit a sample Certificate of Insurance as long as it lists the types and limits the vendor is insured for as required in Section L of the RFQ.
Question 4: Under Section O (Testing Laboratory Qualifications) on Page 4, the RFQ states the following: "Testing shall be conducted by laboratories licensed and/or certified for the testing requested. Bidders shall submit copies of laboratories licensed and certifications with their proposals." Based on our understanding of the scope of work, the services of a laboratory are not required. Please clarify what kind of laboratory the city is looking for.
Answer 4: That is correct, the services of a laboratory are not required. However, should laboratory testing be needed, it shall be conducted by laboratories licensed and/or certified for the testing requested.

Should Subcontractor Affidavits be submitted with our proposal? If so, please provide the form to be used.
Answer 5: Yes. Both contractors and their subcontractors must fill out the Contractor
Affidavit that is provided on page 12 of the RFQ solicitation package.
Question 6: The Cost Proposal states the following: "Lead-Based Paint Testing and Inspection
Services shall be completed within twenty (20) days after notification. Written & Visual Assessment Reports will be completed within ten (10) days following the inspection and assessment of rehabilitative structures. Clearance exam within two
(2) days following completion of testing. Written Assessment will be completed within ten (10) days following clearance inspection." Please clarify whether these these timeframes are "calendar days" or "business days."
Answer 6: These timeframes are business days.
Question 7: In the References/Work History section on page 9 of the RFQ, we are instructed to provide "at least five (5) clients for whom the firm has provided the same or similar services." Should we interpret this to mean that more than 5 clients can be provided?
Answer 7: Vendors may provide more than five (5) clients/references and may photocopy the form for extra space if needed.
Question 8: The List of Subcontractors on page 11 has a column for "Class of Work to be
