About this file

This is Addendum #1 to Request for Proposal 487-26 issued by the State of New Hampshire, Division of Procurement and Support Services, for Pharmacy Benefits Manager (PBM) services to administer prescription drug benefits. The addendum, dated April 8, 2026, addresses eleven clarifying questions submitted by prospective bidders and modifies specific requirements outlined in the original RFP. The procurement covers approximately 26,145 covered lives under the Employee and Retiree Health Benefits Plan for calendar years 2027, 2028, and 2029. Bidders must submit proposals electronically through ProposalTech by April 22, 2026, at 2:00 PM Eastern Time. Key clarifications include confirmation that retail prescriptions are limited to a 31-day supply (with the current contract pricing basis of 1-83 days to remain), specialty drugs are limited to a 30-day supply, and bidders should provide retail 90-day pricing as a supplemental quote for potential future implementation. The State confirmed that biosimilar claims and Average Wholesale Price (AWP) will be shifted to preferred low-cost alternative products to reflect current market dynamics, and that 340B claims should be included in all guarantees rather than excluded.

The financial proposal requires bidders to submit pricing utilizing either pass-through or Acquisition Cost Plus methodologies across multiple distribution channels, with Per Member Per Month (PMPM) guarantees requested in addition to traditional pricing guarantees for ingredient costs, dispensing fees, and rebates. Bidders may submit proposals with either pure PMPM offers or traditional pricing quotes that include PMPM guarantees; both approaches are acceptable. Vaccines (COVID and non-COVID) and prescribed antiviral claims may be excluded from discount, dispensing fee, and rebate guarantees, while OTC COVID test kits are not covered. The State clarified that Section 4.1.1 of the RFP is deleted, and the successful bidder will execute the State's standard P-37 contract form incorporating Exhibits A, B, and C following award notification, with no acceptance of bidder-submitted alternative contract terms. All changes noted in this addendum supersede previously submitted RFP documents and must be included with bid submissions.

STATE OF NEW HAMPSHIRE

DIVISION OF PROCUREMENT AND SUPPORT SERVICES

BUREAU OF PURCHASE AND PROPERTY

STATE HOUSE ANNEX

25 CAPITOL STREET

CONCORD, NEW HAMPSHIRE 03301-6398

Date of Change: April 8, 2026

ADDENDUM # 01 TO BID INVITATION 487-26

DATE AND TIME OF BID CLOSING: 4/22/2026 2:00 PM

FOR: ADMINISTRATION OF PRESCRIPTION DRUGS

Question 1: Confirm days supply limit at Retail (i.e. Retail 30 = 1-31 days and Retail 90 = 31+, etc).

State Response: The State’s plan design allows up to a 31-day supply for retail and up to a 90-day supply is available via mail. However, the current contract guarantees pricing for retail scripts for 1-83 days. The State is requesting retail pricing consistent with the current 1-83 days.

Although the State’s current plan design does not currently include a retail 90 program, the State requests that Bidders provide retail 90 pricing (84+ days) as part of their proposal in the event the State considers implementing this provision in the future. Proposed pricing for retail or mail scripts should be based on the current plan design (without retail 90) and cannot be contingent on the implementation of a retail 90 program.

Please note that a member can opt out of the mandatory mail provision and fill these prescriptions at retail but is still subject to the 31-day supply limit.

For additional details regarding day supply limits please reference the linked plan documents below:

• 2026 Active Plan - Pharmacy Benefits

• 2026 Non-Medicare Retiree Plan – Pharmacy Benefits

State of New Hampshire PBM RFP – Financial Attachment has been updated to reflect the requests summarized above.

Question 2: Confirm days supply limit at Specialty.

State Response: Specialty drugs are limited to a 30-day supply

Question 3: Confirm how biosimilars will be treated in the analysis. Underlying data may show high utilization of Stelara, but many PBMs cover the Low WAC product. Please confirm that you will shift https://www.das.nh.gov/hr/documents/health_benefits/ESI-Benefit-Summary-2026.pdf https://www.das.nh.gov/documents/rmu/retiree/benefits/2026/2026-Retiree-U65-RX-Plan-Summary.pdf https://www.das.nh.gov/documents/rmu/retiree/benefits/2026/2026-Retiree-U65-RX-Plan-Summary.pdf the claims and AWP to the PBM’s preferred low WAC products?

State Response: The analysis will shift claims and Average Wholesale Price (AWP) to the PBM’s preferred low WAC products to accurately reflect current market dynamics and net costs of claims.

Question 4: Confirm for the PMPM guarantee option, that the only guarantee we are quoting is the single number all-in PMPM guarantee. Individual components in the table such as ingredient cost, dispensing fee, and PMPM rebates are not guarantees but are illustrative values in how the PBM build up the PMPM guarantee?

State Response: The State’s requested pricing methodology is pass-through for retail with acquisition cost plus for mail / specialty with the administrative fees and guarantees by distribution channel indicated in the State of New Hampshire PBM RFP – Financial Attachment. In addition, the State is interested in receiving proposals that also include PMPM guarantees, such as a trend guarantee. The PMPM guarantees are requested in addition to (and not instead of) the other guarantees requested.

Question 5: 4.1.1 of the RFP states “Confirm the PBM has attached a signature ready contract incorporating all agreed upon provisions within this RFP and subsequent responses as indicated in this RFP. Please indicate the name of this attachment.” Please confirm whether the State will accept a draft contract incorporating all agreed upon provisions as part of the finalist or upon award, wherein bidder can more fully incorporate agreed upon terms and pricing inclusive of its follow up responses or clarifications.

State Response: Section 4.1.1 is hereby deleted. As noted in Section 2.1.16, “The form contract P-37 (Appendix F) shall form the basis for any resulting contract. The successful Bidder and the State, following notification of award, shall promptly execute the P-37 contract, to include Exhibits A, B, and C referenced in Section 2.1.16. In no event is a Bidder to submit its own standard contract terms and conditions as a replacement for one or all of the State's terms in response to this RFP.”

Question 6: Given financials are the largest part of the score, and if a bidder could only submit one bid, is the State of New Hampshire leaning more towards a pure PMPM offer or would they prefer a traditional pricing offer quote that includes a PMPM guarantee?

State Response: See response to Question #4.

Question 7: The RFP introduction notes that the State requires Bidders to duplicate the current plan design. Please confirm if bidders should quote assuming members cannot fill 90 days at retail and should use mail order instead?

State Response: Confirmed, the State’s current plan design allows for up to a 31-day supply at retail and up-to a 90-day supply is available via mail and does not currently include a 90-day retail option. See response to Question #1.

Question 8: It is noted that Vaccines (COVID) may be excluded from the discount, dispensing fee, and rebate guarantees. Please confirm if COVID test kits and antivirals may also be excluded from the guarantees.

State Response: Vaccines (COVID and non-COVID) and prescribed antiviral claims may be excluded from the discount, dispensing fee and rebate guarantees (see Q5.4.3). OTC COVID test kits are not covered under the State Prescription plan.

Question 9: Please confirm 340B Claims will be allowed as an exclusion for discount, dispensing fee, and rebate guarantees.

State Response: 340B claims should be included in all discount, dispensing fee (see Q5.4.5) and rebate guarantees (see Q5.4.6).

Question 10: Please confirm that the pricing proposal for the commercial population should reflect the Maximum Fair Price (MFP) impact for 2026 only, as the pricing impacts for 2027 have not yet been determined.

State Response: Confirmed, the State is looking for bidders to include/underwrite all known pricing impacts, (e.g., MFP, biosimilars, etc.) as part of its proposal.

Question 11: The State's current Retail 30-day supply basis is 1-83. Based on the Financial Guarantees tab, please confirm the State would like the Retail 30-day supply basis to be 1-31.

State Response: Please see response to Question #1.
