About this file
This is Addendum 1 to an Eyeglasses Request for Proposals (RFP #200000000058) issued by a state agency on October 16, 2019. The addendum addresses four key clarifications regarding the Medicaid Vision Program contract requirements. The modifications clarify policies for lens replacement when diopter changes occur due to lens design changes, establish that replacement lenses must be identical copies of damaged or broken lenses unless prescription changes meet minimum diopter requirements, revise resume submission requirements to include contact information only for work assignments completed within the last five years (or note when unavailable for newer team members), and correct a cross-reference error regarding subcontractor information requirements.
The addendum reflects alignment with current Medicaid policy as outlined in the Medicaid Provider Manual Vision chapter, section 3.4.D. Key modifications include eliminating the prior authorization requirement for lens design changes that meet the minimum diopter criteria specified in section 3(a), while retaining prior authorization requirements for lens design changes accompanied only by add power changes. The revised resume requirements acknowledge the practical challenge of obtaining contact information for individuals with lengthy work histories, limiting the requirement to the last five years of employment. These clarifications were posted on October 16, 2019, with questions and answers due the same date.

1.
Item 3(e)
16
The item states that “if the diopter change is due to a change in lens design (i.e. bifocal to single vision), the Contractor must return the order DCH 0893 to the provider for submission to the (PRD) Vision Program for possible exception to the Medicaid Policy.”
Under the current contract, where a diopter change is due to the change in lens design (from bifocal to single vision or from single vision to bifocal) and meets the minimum Rx diopter change requirement, there is no requirement for the Contractor to return the order DCH 0893 to the provider for submission to the (PRD) Vision Program for possible exception to the Medicaid Policy. This policy changed several years ago.
Please clarify whether the State is seeking to modify current practice and policy and return to when such an Rx change requires prior authorization.
Subsequent lens replacement without PA is allowed if diopter criteria are met as defined in 3(a) which does not include add power change.
This would supersede change in lens design with change in add power only, thus not requiring PA if both diopter criteria from 3(a) and lens design are indicated.
Change in lens design with change in add power only as defined in 3.e requires return of the order for PA.

Item 4(a)
16
This item allows replacement of broken or damaged lenses within the benefit limit as long as the replacement limits have not been exceeded.
While this is an accurate reflection of the current program, there is an additional requirement under the current contract. The current contract requires that the replacement lenses be of the exact same prescription as the original pair, unless there is a change in prescription that meets the minimum diopter change requirement to allow for the replacement.
The State has modified this as follows to align with Medicaid policy as outlined in the Medicaid Provider Manual Vision chapter, section 3.4.D Replacement or Repair:
Replacement of broken or damaged lenses, without frames, is a benefit if the lenses are covered by Medicaid and the replacement limits have not been exceeded. A replacement lens(es) must be an identical copy of the damaged or broken lens.

Last paragraph of Section 3.5
29
This section requires that the resumes of key personnel include contact details for person or persons who may be contacted from each project or work assignment.
Please confirm that the State is seeking contact details for each position held by the individual whose resume is appended. For example, some personnel have been working for forty years. Providing accurate contact details that far back may be impossible for some individuals.
Would the State consider limiting the contact information for those positions held within the last five years? Additionally, if such limitation is offered, is the State looking for contact details for an employee of the bidder who is vouching for another employee of the bidder; this may occur for some of the newer team members of bidder.
Please clarify the State’s wishes.
The published RFP states:
“Resumes must include detailed, chronological work experience, including name and phone number of person or persons who may be contacted from each project or work assignment.“
The State has modified this as follows:
Resumes must include detailed, chronological work experience, and if available, may includeing name and phone number of person or persons who may be contacted from each project or work assignment completed within the last 5 years.
If phone reference phone number or name are not available due to a newer team member being proposed, the bidder is not requested to provide alternative employee contact details.
