About this file

This document is an addendum to a Request for Statements of Qualifications (RFSQ) issued by the Florida Department of Health for Architecture-Engineering Continuing Contracts for the Northern Region of Florida. The addendum provides responses to questions submitted by interested parties and revises the submittal deadline to August 8, 2022 before 4:00 PM EST. The RFSQ seeks to establish continuing contracts with design professional teams capable of providing architectural, engineering, and related services for projects in the Northern region of Florida, which includes Leon County and the City of Tallahassee. The original maximum page count for submissions has been increased from 25 to 50 pages.

The addendum also clarifies that the commodity codes listed in the RFSQ are used by the agency to target a refined set of prospective applicants, and do not define the specific scope of services. Additionally, the agency will not be activating service providers under this agreement strictly for construction administration, as that is considered a basic service. The agency has provided options for submitting qualifications packages that exceed the email file size limit, including mailing a thumb drive, uploading to a secure site, or submitting a printed copy.

ADDENDUM #1

Advertisement Number: RFSQ-01935 Agency Project No. 20221000

Architecture–Engineering Continuing Contracts for North Florida Region DATE: July 25, 2022

The following are all questions and responses received by the Agency through July 25, 2022. The information in the responses supersedes the information in the initial advertisement package.

Questions Submitted Friday, 7/15/2022

1. The team we have assembled so far includes, Architectural, Landscape Architectural, Structural engineering, Plumbing, Electrical, and Mechanical Engineering, and Civil Engineering. Please provide additional descriptions for commodity codes noted on the advertisement, noted bellow? We assume commodity code 72121100 indicates our team to provide construction administration.

81101508 Architectural engineering

81101523 Urban design and engineering service

72121100 Commercial and office building construction services

RESPONSE:

A) Construction administration is part of the basic services to be provided by a team when assigned a design project. We will not activate a service provider under this agreement strictly for construction administration.

B) The commodity codes are utilized by the Agency to direct the advertisement towards a refined set of perspective applicants and do not define the specific scope of services.

2. Is there another option to forward our final RFQ to you if the RFQ size exceeds the maximum accepted by our email provider?

RESPONSE:

A) Please contact Kathryn.Vowell@Flhealth.gov to discuss the options available if you are unable to transmit your RFQ package via email due to file size. The only options available are:

1. Submitting a thumb drive via mail delivery

2. Uploading to a secure OneDrive site set up by the Agency

3. Submitting a printed copy via mail delivery.

B) The Department of Health internet security protocols do not allow access to commercial file share sites, corporate FTP sites, or OneDrive sites outside of State government. RFQ packages uploaded to such sites are nonresponsive.

Questions Submitted Tuesday, 7/19/2022

1. Would the DOH consider increasing the page limit from 25 pages to 40 pages?

We realize that Department of Health does not desire extraneous information. We basing or request on the following:

- Cover letter: 1-2 pages

- the PQS is 5 pages

- the SF330 including Part II: 24-29 pages Providing 5-10 projects, resumes for all major disciplines, and Part II for outside consultants

- Copy of our licenses will be 1 page

RESPONSE: The maximum page count will be increased from twenty-five (25) to fifty (50) pages. All pages after 50 shall be marked NOT APPLICABLE and will not be considered in the review process.

2. Is it possible to receive a PQS in Word or Excel to allow easier completion?

RESPONSE: Please contact Kathryn.Vowell@flhealth.gov to receive a Word file template of the PQS.

Question Submitted Friday, July 15, 2022 We are writing Re: RFSQ-01935 Architecture-Engineering Continuing Contracts for North Florida to ask if the 25-page limit has to be single sided or if they can be double-sided?

RESPONSE: Single sided. Per the instructions, all submittals will be via PDF. There are no single or double-sided sheets.

Question Submitted Wednesday, July 13, 2022 I noticed the continuing contracts advertisement for the DOH and wanted to know if your team was in need of 3D laser scanning for digital twin creation and field verification? With the digital twin, functional teams can collaborate through an interactive web-viewer and PDF floorplans, CAD drawings and 3D models can also be generated, accurately from the existing conditions.

Recently, we scanned the Lake Butler Hospital in Lake Butler, Florida to create life safety drawings, renovations and facility management. If possible, I would like to schedule a demo with you and your team to show how we can help improve facilities and future projects. If you would like a link to the 3D web viewer, please let me know.

RESPONSE:

Per the FAQ, we are soliciting for full-service design professional teams, not individual subconsultant providers.

Question Submitted Saturday, July 23, 2022 There is duplicate information requested by DOH's PQS form and the SF330. Duplicate information includes:

1. Items 1 and 2 of the PQS and Part 1 a, b, and c of the SF330

2. Item 6 of the PQS form and Section F of the SF330

If we submit both forms in their entirety, they would occupy approximately 28 pages, which is over the stated limit of 25 and does not allow room for any other required content. Will DOH consider raising the page limit and omitting duplicate form sections?

RESPONSE:

A) The maximum page count will be increased from twenty-five (25) to fifty (50) pages.

B) The applicant will include all requested information in both the PQS and SF330. They are two separate documents and information cannot be cross referenced.
