About this file
This document is an Addendum to an Invitation to Bid (ITB) issued by the Commonwealth of Kentucky Department for Facilities and Support Services, Division of Engineering and Contract Administration for an HVAC Piping Replacement project at Western State Hospital in Hopkinsville, Kentucky. The Addendum extends the bid due date to December 6, 2022, and includes several revisions to the contract documents. Key changes include revised COVID-19 vaccination requirements for the contractor's unvaccinated employees, the addition of a new specification section on Mechanical Insulation, and clarification that wage rates do not apply to this project.
The Addendum also includes attachments for a Vaccine Exemption Form and the new Mechanical Insulation specification section. The project appears to be isolated from staff and patients, with unvaccinated contractor employees required to sign an exemption form, undergo daily temperature checks and weekly COVID testing provided by the hospital. This HVAC piping replacement project does not have any wage rate requirements.
