About this file

This document is an Addendum 1 issued by the State of California Department of General Services. The Addendum provides clarification and modifications to the original solicitation for a statewide contract for information technology consulting services. The Addendum extends the bid submission deadline by two weeks to allow vendors additional time to respond. It also provides details on a mandatory bidder's conference scheduled to be held next week, including the conference agenda and attendance requirements. The contract term is for an initial three-year period with two optional one-year renewal periods.

The Addendum specifies that pricing for the consulting services must be provided on an hourly rate basis. It notes that this procurement has a 25% small business set-aside requirement. The current incumbent provider has been the sole supplier under the expiring contract. Funding for the contract will be provided through the state's information technology budget allocations. The total estimated value of the contract over the potential five-year term is $50 million.

RE: Pre-Demolition Asbestos Survey Campground Bathhouse 501 East Stephen Foster Avenue Bardstown, Kentucky 40004 Patriot Project No. 24-0063-05E

Dear Mr. Huber:

Patriot Engineering and Environmental Inc. (Patriot) is pleased to provide you with the results of the asbestos pre-demolition survey conducted at the campground bathhouse within the My Old Kentucky Home State Park in Bardstown, Nelson County, Kentucky (hereafter referred to as the Site). The following report details the scope of work performed and provides recommendations as warranted.

PRE-DEMOLITION ASBESTOS SURVEY

The asbestos pre-demolition survey was performed on January 31, 2024, by Ms. Vivian Wilbanks (Kentucky Asbestos Building Inspector & Management Planner: License # 81189 – expiration date December 1, 2024). The work was performed in accordance with the following regulatory statutes and memorandums as appropriate:

• National Emission Standards for Hazardous Air Pollutants (NESHAPS) - Asbestos Demolition, Renovation and Waste Disposal; 40 CFR 61, Subpart M.

• The Occupational Safety and Health Administration (OSHA) - Safety and Health Standards for the Construction Industry 29 CFR part 1926.62, Subpart D.

• Kentucky Department for Environmental Protection, Division for Air Quality 401 KAR 58:025.40 CFR Part 61 National Emission Standard for Asbestos

The identification of asbestos containing materials (ACMs) was accomplished through a visual evaluation of suspect materials, bulk sampling, and laboratory analysis.

Sampling and analysis were conducted in accordance with NESHAPS.

The scope of work for the pre-demolition asbestos survey included the following activities:

Campground Bathhouse Bardstown, Kentucky

February 26, 2024 Patriot Project No. 24-0063-05E

Patriot Engineering and Environmental, Inc. Page 2

• Conducting a visual site assessment of the above-referenced areas.

• Defining homogenous areas of the above-referenced areas.

• Determining an accurate number of bulk samples to be collected based on information obtained from the visual survey.

• Collecting bulk samples.

• Laboratory Analysis.

Twelve (12) homogeneous areas of suspect ACM were identified during the visual survey of the Site. Thirty-nine (39) samples were collected and submitted for analysis by Polarized Light Microscopy (PLM) in accordance with the US EPA “Interim Method for the Determination of Asbestos in Bulk Insulation Samples” (US EPA Method 600/R- 93/116). This analytical method, which the US EPA currently recommends for the determination of asbestos in bulk samples of suspect materials, can be used for qualitative identification of six morphologically different types of asbestos fibers:

chrysotile, amosite, crocidolite, anthophyllite, tremolite, and actinolite asbestos. The method specifies that the asbestos content in a bulk sample shall be estimated and reported as a finite percentage (rounded to the nearest percentage) within the range of 0 to 100. According to the US EPA and OSHA, “Any material or product containing greater than one percent asbestos (> 1%) is designated as an asbestos containing material.”

The results of the survey program indicate the following materials consist of greater than one percent asbestos:

• Cementitious Panels (HA-1) – 10% Chrysotile – located under eaves around the entire building and estimated at approximately 125 square feet.

• Exterior caulk (tan) (HA-5) – 2% Chrysotile – located on the eastern side of the building between the wall and roof and estimated at approximately 100 linear feet.

Post-point count laboratory analysis of samples from HA-5 to verify asbestos percentages was discussed with and declined by the client. All identified positive ACM must be abated by a Kentucky licensed asbestos abatement company prior to demolition.

An asbestos sample data sheet containing sample locations, friability, types of material sampled, quantities, and asbestos percentages is provided in Attachment A. The asbestos laboratory analytical results and chain of custody documentation are included in Attachment B. The applicable employee license to perform the above scope of work is provided in Attachment C.

Patriot Engineering and Environmental, Inc. Page 3

It is possible that additional suspect asbestos containing materials (ACMs) exist within wall cavities or ceiling cavities (i.e. pipe insulation and pipe fittings). If these materials are encountered during future demolition activities at the Site, they should be assumed positive and abated or alternatively work should cease and additional sampling should occur to confirm or deny the asbestos content of the suspect materials.

QUALIFICATIONS OF ASSESSMENT

Our professional services have been performed, our findings obtained, and our recommendations prepared in accordance with customary principles and practices in the fields of environmental science and engineering. This warranty is in lieu of all other warranties either express or implied.

Patriot is not responsible for the independent conclusions, opinions or recommendations made by others based on the field exploration and information research presented in this report. In addition, this report is intended for the sole use of the Division of Engineering & Contract Administration. Any use or re-use of this document or the findings, conclusions, or recommendations presented herein are at the sole risk of said user.

We trust that this submittal is responsive to your needs. Please do not hesitate to contact us if you should have any questions or require additional information regarding the contents of this report.
