About this file

This document is an Addendum to the Invitation to Bid (ITB) issued by the Florida Department of Transportation (FDOT) District Six for the District-Wide Right-of-Way Asbestos and Air Monitoring Consulting Services in Miami-Dade and Monroe Counties. The ITB, DOT-ITB-23-6181IZ-Rebid, is seeking proposals to provide these services. The estimated quantities for the various line items range from 10 to 40 units. The proposal due date is September 6, 2023, at 10:00 AM, via email to the FDOT Contracts Office. The addendum addresses questions from prospective proposers and provides clarification on the Certified General Contractor requirement, the unit pricing, and the overtime/shift rate. It also notes that the spreadsheet Bid Price Proposal has been updated with formulas and included with the addendum.

Regarding pricing, the unit prices should be entered, and the total amounts will be calculated accordingly. The addendum also states that the sample forms, such as the Performance Bond, Contract Affidavit, and Standard Written Agreement, do not need to be included in the Bid Price Proposal package and will be required from the awarded vendor.

Work Description: To provide District-Wide Right-of-Way Asbestos and Air Monitoring Consulting Services in Miami-Dade and Monroe Counties for the Florida Department of Transportation (FDOT)

Question 1: On page 7 of the Invitation to Bid Doc, Section 9 Qualifications, #4 states “The prime bidder/proposer shall provide proof that the firm is certified with the State of Florida as a

Certified General Contractor with a Division 1 (a) License.”, however, this ITB is for

Asbestos Consulting. Therefore, is it truly a requirement to be a CGC to be qualified for this ITB?

Answer 1: A “Certified General Contractor” with a Division 1 (a) License is required to be qualified for this Project (ITB).

Question 2: Section 1 asks for a “Unit Price” and indicates the “Unit” as Per Square Foot, however, indicates estimated units to be 40 for items 1.1, 1.2 & 1.3 and 10 for 1.4 & 1.5. Is the

Department asking for an estimated price for a 40 or 10 square foot building or for 40 or

10 buildings for items 1.1 – 1.5?

Answer 2: Size of building (sf) x Unit Price x 40 Units.

Question 3: Item 3.2 Work Description is “Assessment-Overtime Rate (Shift Rate)” then indicates the

“Unit” as Per Plan with an estimated 10 units. Should this be the overtime hourly rate? Or the shift rate with x hours of overtime? If this is for a shift rate with x hours of overtime, how many hours of overtime should be used to estimate the cost?

Answer 3: Yes, it is the overtime hourly rate.

Question 4: I want to make sure that the forms listed below are “sample only” and they do not have to be included with our bid package in section B) for Required Forms?

Performance bond form (375-040-27) Contract Affidavit (form 375-020-30) Standard written agreement (form 375-040-19)

Answer 4: The forms above do not have to be included in the Bid Price Proposal package. They will be required by the awarded vendor.

Question 5: There are numerous cells throughout the Bid Price Proposal file that appear to be missing formulas – see sample snip below (unit prices entered as examples). Will the

FDOT issue a revised bid sheet, or shall we enter unit prices and leave the totals as-is?

Answer 5: The spreadsheet Bid Price Proposal has been updated with the formulas and included with this addendum.

All other documents of the subject project remain unchanged.
