About this file

This Addendum #1 document issued by Door County, Wisconsin on August 13, 2019, provides clarifications and responses to questions regarding Request for Proposal No. DC-SRT-2019 for Shared Ride Taxi Service. The proposal deadline is September 6, 2019 at 2:00 PM. The service contract will operate from January 1, 2020 through December 31, 2022, with two optional one-year renewal periods. Bidders must acknowledge receipt of this addendum by signing and submitting it with their proposal to be considered responsive. All other terms of the original RFP remain unchanged unless specifically modified in this addendum.

Key clarifications from the question-and-answer section address proposal submission requirements and service parameters. The 25-page, double-sided limit excludes appendices, financial statements, vehicle maintenance plans, price proposals, affidavits of non-collusion, and the table of contents. Proposers must submit Appendices A (Affidavit of Non-Collusion), D (Federal Required Clauses and Certifications), F (Designation of Confidential and Proprietary Information if applicable), and G (Bid Opportunity List). The Door 2 Door Rides program does not operate on six major holidays—New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas—and does not provide service for special events. Selected contractors must submit an annual "State of Wisconsin Single Audit Guidelines Report" but will not be required to provide a "Schedule of Revenue and Allowable Cost by Contract." The Price Proposal (Appendix A) must be submitted as a separate sealed document with proper marking per RFP instructions.
