About this file

This is an Addendum #1 Bid Sheet for a grave relocation project in Magoffin County, Kentucky, involving the Federal Highway Administration (FHWA) Emergency Relief program. The project requires a qualified funeral director to manage the relocation of multiple graves to Prater-Preston Cemetery. The scope of work includes disinterment of remains for Ivy Jean Brown, William Brown, and Guy Douglas Brown; provision of grave markers with lettering and setting; transport of remains across five miles; reinterment of remains; transport and reset of the Brown Monument; and ground clearance and brush removal as needed. The awarded contractor will be responsible for opening and closing reinterment plots, seeding and strawing, pouring monument footers, and setting monuments. Vendors must bid all 15 line items to be considered for award, and prospective bidders are encouraged to contact Amy Ishmael at 606-291-9641 to view the disinterment and reinterment cemetery locations prior to submission, though no mandatory on-site meeting will be held.

All remains are reportedly held in vaults according to next of kin; however, the awarded funeral director must be prepared to replace any vault if deterioration or damage is discovered during the process, with costs for vault replacement subject to contract modification. The bidder must supply all necessary equipment for both the disinterment and reinterment cemetery locations. Bid prices should be entered directly on the bid sheet and uploaded into the e-procurement system, with all line items currently showing $0.00 as placeholders pending vendor submission of actual pricing.
