About this file

This Addendum 1 document is issued by the New York State Office for People with Developmental Disabilities (OPWDD) in response to vendor questions regarding the 2nd Floor Bathroom Renovation project at the Bernard Fineson DDSO facility located at 115-29 217th Street in Cambria Heights, NY. The project involves comprehensive bathroom renovation work requiring contractors to furnish all fixtures, including vanity, toilet, lighting, and shower components, with the scope encompassing removal of existing fixtures, installation of new plumbing and electrical systems, tiling, and implementation of ADA accessibility features. The original solicitation date was April 25, 2025, with quotes due by 2:30 PM on June 11, 2025, and mandatory site visits scheduled for May 6, 2025. The project must be completed within 14 days of award, with all work performed between 9:00 AM and 3:00 PM, Monday through Friday, to minimize disruption to residents in this occupied facility.

The addendum clarifies that submittals and shop drawings are not required; however, product data sheets and samples must be provided before ordering. Architectural drawings and plumbing risers will not be provided by OPWDD, and no material staging area is designated on the property. All existing plumbing must be replaced per the scope regardless of condition, and OPWDD will not supply any fixtures or components. The document confirms that change orders may be submitted if unforeseen conditions such as mold or structural damage are discovered and verified by OPWDD's team, and vendors should account for this possibility in their estimates. No specific budget range is provided in the addendum, though the related RFQ tally sheet indicates bid amounts ranging from $21,624.80 to $64,860.00. Contractors must maintain $1 million in liability insurance, be registered with the New York State Department of Labor, comply with prevailing wage requirements under PRC number 2025005030, and provide a three-year warranty on all work.
