About this file

This is a Vendor Accessibility Disclosure Form issued by the Colorado Community College System (CCCS) to ensure compliance with accessibility standards for information and communication technology. The form applies to websites, instructional materials, online services, software applications, multimedia content, and kiosks developed or maintained by CCCS entities. Vendors are required to disclose information about up to two products or applications, including their names, versions, and release dates. The form does not apply to unauthorized web pages published by students or employees or to third-party content linked to CCCS websites unless such content allows public participation in CCCS services, programs, or activities. Vendors must certify the accuracy of all information provided and acknowledge that false statements may disqualify them from the procurement process.

Vendors must indicate whether their products or applications are Web Content Accessibility Guidelines (WCAG) 2.1 Level AA compliant, are not compliant but working toward compliance, or are not compliant and not pursuing compliance. If either of the first two compliance categories applies, vendors must submit a completed VPAT 2.5 WCAG document with the disclosure form. The form requires vendor contact information, including name, address, phone number, email, and any doing-business-as names. This disclosure mechanism aligns with CCCS Board Policy 3-125 (Electronic Communication Policy) and Standard Procedure 19-60c (Digital Accessibility), reflecting the system's commitment to Universal Design principles and accessibility standards established by the Governor's Office of Information Technology.
