About this file
This is a Contractor Conflict of Interest Disclosure Form (ADAP Form 209-506-1, August 2024) template required for the Cybersecurity Support Services Bridge Contract (Solicitation Number HQ003426QE115) with the Department of Defense Washington Headquarters Service (DSCA). The form serves as a mandatory disclosure document that contractors must complete to identify and address any actual or potential conflicts of interest related to the solicitation or contract performance.
The form requires contractors to provide comprehensive information across fourteen blocks, including the WHS solicitation or contract number, program supported, organization details, point of contact information, and a detailed description of the work to be performed. Contractors must disclose whether actual or potential conflicts of interest exist and, if so, provide specific information about related solicitations, contracts, or subcontracts, including contract numbers, program names, periods of performance, and contract values. The form mandates detailed narratives explaining the relationship between the proposed work and any prior related work, followed by an organizational conflict of interest evaluation and mitigation plan. For personal conflicts of interest, contractors must conduct a thorough analysis of governing body members, principals, managers, and key personnel, examining their assets, income sources, outside positions, liabilities, and any healthcare-related arrangements. The form must be completed separately for each related solicitation, contract, or subcontract and signed by a responsible company official with authority to address organizational conflict of interest matters.
