About this file

This is a Performance Work Statement for the relocation of mission-critical medical equipment and medical records storage from Building 34 to Building 46 during a renovation project. The project encompasses three primary work categories: relocating 2 Panoramic Dental X-Ray Machines within Building 34 to areas not undergoing modernization renovation; disassembling and relocating 8 Medical Record Storage Systems within Building 34 to unaffected areas; and disconnecting, dismounting, and relocating 3 immunization fridges and 1 sensiphone from Building 34 to Building 46's kitchen area. All work must be performed during regular business hours with contractor collaboration with the customer to ensure proper placement.

The contractor is responsible for performing all relocation tasks, ensuring compliance with current building and fire codes, completing or subcontracting any required electrical work for the sensiphone, and finalizing relocations with the customer. The customer must clear existing items from both buildings to ensure proper spacing, designate storage locations, and provide building access during regular business hours. The critical deadline is no later than 28 March 2026 for equipment removal to prevent interference with the Building 34 remodel project start date. Acceptance criteria require all relocations and reassemblies to meet current building and fire codes, all electrical work to be completed and functional, and all equipment to be relocated to designated areas per the agreed-upon design. The budget will be determined based on the scope of work and mutually agreed upon by both parties prior to work commencement. Specific project timelines will be collaboratively established between the customer and contractor before work begins.
