About this file
This document is a Small Business Subcontracting Plan Template for a medical services contract (RFP/Contract No: 1605C3-25-R-00004) with the Department of Labor's Office of Workers' Compensation Programs. The template outlines subcontracting goals across six small business categories for the entire contract period, with consistent percentage allocations across base and option periods: Small Business (60%), Small Disadvantaged Business (11%), Women-Owned Business (7%), HUBZone Business (3%), Veteran-Owned Small Business (5%), and Service-Disabled Veteran-Owned Small Business (5%).
The contract is for Medical Second Opinion and Examination Services supporting the Federal Employees' Compensation Act (FECA) program, requiring board-certified, licensed physicians to perform impartial medical second opinion evaluations, referee medical examinations, and related administrative services for claimants across the United States and its territories. The subcontracting plan allows for overlap in small business categories, meaning a single subcontractor could be counted in multiple classification types if they meet multiple small business certification criteria.
